摘要
目的 分析乳腺导管原位癌(DCIS)的超声与临床病理特征。方法 选取昆山市第二人民医院2018年1月至2022年12月收治的DCIS以及乳腺导管内原位癌伴微浸润(DCISM)患者共150例作为研究对象。对比分析DCIS以及DCISM患者的超声以及临床病理特征的差异,分析两者之间的相关性。结果DCIS以及DCISM患者超声表现差异无统计学意义(P>0.05),但DCIS患者的病灶范围更小,微钙化情况较少,同时存在微钙化的患者较无钙化患者的病灶范围更大,病理分级、人表皮生长因子受体2(HER2)阳性率以及Ki-67增值指数更高,雌激素受体(ER)以及孕激素受体(PR)阳性率更低(P<0.05)。在病理方面,DCISM患者的病理分级、ER以及PR阳性例数、HER2阳性率以及Ki-67增值指标明显高于DCIS患者(P<0.05)。结论超声作为一项临床辅助诊疗手段,利用其在诊断方面的影像学特点和优势,加大在DCIS筛查和诊断方面的力度,做到对DCIS患者早发现、早诊断、早治疗,对DCIS及DCISM患者的检出、诊疗及预后评估有重要的临床意义。
Objective To analyze and summarize the ultrasound and clinicopathological characteristics of ductal carcinoma in situ of the breast(DCIS).Methods A total of 150 patients with DCIS and intraductal carcinoma in situ with microinvasion(DCISM)admitted to Second People′s Hospital of Kunshan between January 2018 and December 2022 were selected for the study.The differences in ultrasond as well as clinicopathological features of patients with DCIS as well as DCISM were compared and analyzed for correlation.Results In terms of ultrasound,there was no significant difference in the ultrasound performance between DCIS and DCISM patients(P>0.05),but DCIS patients had a smaller range of lesions and fewer microcalcifications,and patients with microcalcifications had a larger range of lesions than patients without calcifications,with higher pathological grades,HER2 positive rates and Ki-67 value-added index,and lower positive rates of ER and PR(P<0.05).In pathology,pathological grade,ER and number of PR positive cases,HER 2 positive rate,and Ki-67 value-added index were significantly higher in DCISM than in DCIS patients(P<0.05).Conclusion Ultrasound,as a clinical auxiliary diagnosis and treatment method,uses its imaging characteristics and advantages in diagnosis and increasing its efforts in the screening and diagnosis of DCIS,so as to achieve early detection,diagnosis and treatment of important clinical significance for DCIS patients,as well as the detection,diagnosis and treatment and prognosis evaluation of DCIS and DCISM patients.
