摘要
目的:分析第一、二届国医大师治疗肝胆病证的处方用药规律。方法:查阅《国医大师专科专病用方经验(第1辑)脾胃肝胆病分册》《国医大师专科专病用方经验(第2辑)脾胃肝胆病分册》中收录的国医大师治疗肝胆病证的处方,通过古今医案云平台对用药规律进行统计分析。结果:共纳入30位国医大师治疗肝胆病证的处方207份,包含中药339味,累计使用频数为2 286。其中出现频数≥20的药物共27味,排名前5位的药物依次为柴胡、茯苓、茵陈、郁金、甘草;药物功效分类排前3位的依次为清热解毒药、利胆退黄药、疏肝解郁药;药物以苦微寒药、甘温药为主,多归脾、肝、胃经。对27味高频药物进行关联规则分析,共得到21条关联规则,常用健脾祛湿药物组合为白术-茯苓,疏肝解郁药物组合为柴胡-白芍,清肝利胆药物组合为郁金-柴胡。聚类分析将药物聚为3类,将边权重设置为>25进行复杂网络分析,提取国医大师治疗肝胆病证的核心组方为:柴胡、郁金、茯苓、白芍、白术、甘草、枳壳、茵陈、泽泻、黄芩、香附、当归。结论:第一、二届国医大师治疗肝胆病证注重顾护脾胃,突出“未病先防,已病防变”的临证思想,多选用补虚药和理气药,并根据湿热、瘀血等不同兼夹证配伍清热药、活血化瘀药、利水渗湿药。
Objective:To analyze the medication rules of the first and second master of Chinese medical in the treatment of liver and gallbladder diseases syndrome through data mining.Methods:Consulted the prescriptions of Chinese medical masters for treating liver and gallbladder diseases syndrome included in the Experience in Prescription Application of Traditional Chinese Medicine Masters in Specialized Diseases(Part 1):Spleen,Stomach,Liver,and Gallbladder Diseases Volume and Experience in Prescription Application of Traditional Chinese Medicine Masters in Specialized Diseases(Part 2):Spleen,Stomach,Liver,and Gallbladder Diseases Volume,and statistically analyzed the medication rules through the cloud platform of ancient and modern medical records.Results:A total of 207 prescriptions for the treatment of liver and gallbladder disease syndrome by 30 Chinese medical masters were included,including 339 traditional Chinese medicines,with a cumulative frequency of use of 2286.There were a total of 27 drugs with a frequency of≥20,and the top 5 drugs in order were Chaihu(Bupleuri Radix),Fuling(Poria),Yinchen(Artemisiae Scopariae Herba),Yujin(Curcumae Radix),Gancao(Glycyrrhizae Radix).The top 3 drugs in terms of efficacy classification were heat clearing and detoxifying drugs,cholagogic and jaundice reducing drugs,and liver soothing and depression relieving drugs.The medicine mainly consists of bitter and slightly cold herbs and sweet and warm herbs,mostly directed towards the spleen,liver,and stomach meridians.Through association rule analysis on 27 high-frequency drugs,a total of 21 association rules were obtained.The commonly used combination of spleen strengthening and dampness removing drugs was Baizhu(Atractylodis Macrocephalae Rhizoma)-Fuling(Poria),the combination of liver soothing and depression relieving drugs was Chaihu(Bupleuri Radix)-Baishao(Paeoniae Radix Alba),and the combination of liver clearing and gallbladder promoting drugs was Yujin(Curcumae Radix)-Chaihu(Bupleuri Radix).Cluster analysis clustered the drugs
