摘要
目的分析间歇蓝光照射联合静脉滴注白蛋白治疗新生儿病理性黄疸的临床效果。方法抽取2017年1月至2019年12月驻马店市第一人民医院收治的新生儿病理性黄疸患儿78例,按照随机数字表法分为对照组与研究组,每组39例。对照组采用间歇蓝光照射治疗,研究组在对照组治疗基础上加用静脉滴注白蛋白治疗。比较两组临床疗效、血清胆红素[总胆红素(TBIL)、直接胆红素(DBIL)、间接胆红素(IBIL)]水平、γ-谷氨酰转移酶(γ-GT)、游离脂肪酸(FFA)、总胆汁酸(TBA)、碱性磷酸酶(AKP)、胆红素恢复至正常时间、住院时间及不良反应发生率。结果研究组总有效率(94.87%,37/39)高于对照组(71.79%,28/39),差异有统计学意义(χ^(2)=7.48,P=0.006)。治疗后,研究组TBIL、DBIL、IBIL水平均低于对照组(P均<0.05)。治疗后,研究组γ-GT、FFA、TBA、AKP水平低于对照组(P<0.05)。研究组胆红素恢复至正常时间和住院时间短于对照组(P<0.05)。研究组呕吐发生率(5.13%,2/39)、腹泻发生率(2.56%,1/39)、脱水(5.13%,2/39)、体温上升(0,0/39)等不良反应发生率均低于对照组(23.08%,9/39;20.51%,8/39;23.08%,9/39;15.38%,6/39),差异均有统计学意义(P均<0.05)。结论间歇蓝光照射联合静脉滴注白蛋白治疗应用于新生儿病理性黄疸中疗效显著,能降低胆红素水平,下调γ-GT、FFA、TBA、AKP表达,加快患儿恢复,且不良反应发生率低。
Objective To analyze the clinical effect of intermittent blue light-emitting combined with intravenous albumin infusion in the treatment of neonatal pathological jaundice.Methods A total of 78 neonates with pathological jaundice treated in Zhumadian First People’s Hospital from January 2017 to December 2019 were selected,and they were divided into control group and study group according to the random number table method,with 39 cases in each group.The control group underwent intermittent blue light-emitting,while the study group treated by intravenous albumin infusion based on the treatment of the control group.The clinical efficacy,levels of serum bilirubin,including total bilirubin(TBIL),direct bilirubin(DBIL)and indirect bilirubin(IBIL),γ-glutamyl transferase(γ-GT),free fatty acid(FFA),total bile acid(TBA),alkaline phosphatase(AKP),time of bilirubin returning to normal,length of hospital stay and incidence of adverse reactions were compared between the two groups.Results The total effective rate of the study group(94.87%,37/39)was higher than that of the control group(71.79%,28/39),and the difference was significant(χ^(2)=7.48,P=0.006).After treatment,the levels of TBIL,DBIL and IBIL in the study group were lower than those in the control group(all P<0.05).After treatment,the levels ofγ-GT,FFA,TBA and AKP in the study group were lower than those in the control group(allP<0.05).The time of bilirubin returning to normal and length of hospital stay in the study group were shorter than those in the control group(P<0.05).The incidence of vomiting,diarrhea,dehydration and hyperthermia in the study group were 5.13%(2/39),2.56%(1/39),5.13%(2/39)and 0(0/39),respectively,which were lower than those in the control group(23.08%,9/39;20.51%,8/39;23.08%,9/39;15.38%,6/39),and the differences were significant(allP<0.05).Conclusions The application of intermittent blue light-emitting combined with intravenous albumin infusion in the treatment of neonatal pathological jaundice has a significant effect.It can reduce the bi
