摘要
目的探讨乳腺癌改良根治术后发生上肢淋巴水肿的危险因素。方法本研究采用回顾性分析法。抽取2020年至3月至2021年3月于郑州大学第一附属医院行乳腺癌改良根治术的患者91例,其中术后发生上肢淋巴水肿的54例患者纳入发生组,另未发生上肢淋巴水肿的37例患者纳入未发生组。查阅数据库相关资料,筛选乳腺癌改良根治术后发生上肢淋巴水肿的危险因素,收集并比较两组患者的相关临床资料,将差异有统计学意义的因素带入Logistic回归方程,分析发生上肢水肿的危险因素。结果两组临床资料比较比较结果显示,两组年龄、体质指数(BMI)、淋巴结清扫范围、清扫淋巴结数目、放疗情况比较,差异有统计学意义(P<0.05);Logistic回归分析发现,高龄、BMI高、淋巴结清扫范围广、清扫淋巴结数目多、进行放疗均是发生上肢淋巴水肿的危险因素。结论高龄、BMI高、淋巴结清扫范围广、清扫淋巴结数目多、进行放疗均是乳腺癌改良根治术后发生上肢淋巴水肿的危险因素,对可能出现上肢水肿的高风险患者应采取相应的预防措施。
Objective To investigate the related risk factors of occurrence of lymphedema in upper limbs after modified radical mastectomy for breast cancer.Methods This study was a retrospective analysis.A total of 91 patients with breast cancer who underwent modified radical mastectomy in the First Affiliated Hospital of Zhengzhou University from March 2020 to March 2021 were selected;among them,54 patients with postoperative lymphedema in upper limbs were included in the occurrence group,while 37 patients without lymphedema in upper limbs were included in the non occurrence group.The relevant data collected in the database were consulted to screen the risk factors of occurrence of lymphedema in upper limbs after modified radical mastectomy for breast cancer.The clinical data of two groups were collected and compared.The factors with statistically significant differences were brought into the Logistic regression equation to analyze the risk factors for upper limb edema.Results The comparison of clinical data between the two groups showed that there were a statistically significant differences in age,body mass index(BMI),lymph node dissection range,number of lymph nodes dissected,and radiotherapy status(P<0.05).Logistic regression analysis showed that older age,high BMI,wide lymph node dissection range,large number of lymph nodes dissected,and radiotherapy were risk factors for occurrence of lymphedema in upper limbs.Conclusions Older age,high BMI,wide range of lymph node dissection,large number of lymph nodes dissected,and radiotherapy are the risk factors for lymphedema in upper limbs after modified radical surgery for breast cancer.Corresponding preventive measures should be taken for patients with high risk of upper limb edema.
