摘要
目的:探讨与分析经阴道彩色多普勒超声与核磁共振成像(MRI)在子宫内膜癌(EC)患者诊断中的应用价值。方法:选择2018年1月~2022年12月在本院拟诊断为EC患者90例作为研究对象,所有患者均给予经阴道彩色多普勒超声与MRI检查。同时所有患者均给予病理检查,并以此为诊断的金标准,判断价值结果。结果:90例患者的经阴道彩色多普勒超声与MRI检查的图像清晰度评分都在6分以上,不同检查方法的图像清晰度评分对比无明显差异(P>0.05)。在90例患者中,病理诊断为EC56例,其中Ⅰ期36例,Ⅱ期14例,Ⅲ期6例。经阴道彩色多普勒超声诊断为EC53例,MRI诊断为EC55例,经阴道彩色多普勒超声与MRI对EC的诊断敏感性分别为92.9%(52/56)、98.2%(55/56),特异性分别为97.1%(33/34)、100.0%(34/34)。经阴道彩色多普勒超声诊断为ECI期32例,Ⅱ期15例,Ⅲ期6例。MRI诊断为ECI期35例,Ⅱ期14例,Ⅲ期6例,经阴道彩色多普勒超声与MRI对EC临床分期的诊断准确性分别为95.6%(86/90)、98.9%(89/90)。ROC曲线显示经阴道彩色多普勒超声与MRI在EC患者的诊断曲线下面积分别为0.858、0.929。结论:经阴道彩色多普勒超声与MRI显示EC的图像清晰度都比较高,对于EC患者的诊断与临床分期判断均具有很好的价值。
Objective:To explore and analysis the application values of transvaginal color Doppler ultrasound and magnetic resonance imaging(MRI)in the diagnosis of endometrial cancer(EC)patients.Methods:From January 2018 to December 2022,90 cases of patients with endometrial cancer planned to be diagnosed in our hospital were selected as the study subjects,and all patients underwent transvaginal color Doppler ultrasound and magnetic resonance imaging examination.At the same time,all patients were subjected to pathological examination and used as the gold standard for diagnosis to determine the value of the results.Results:The image clarity scores of transvaginal color Doppler ultrasound and magnetic resonance imaging in 90 patients were above 6 points,and there were no significant difference in the image clarity scores compared between different examination methods(P>0.05).In the 90 patients,pathological diagnosis were endometrial cancer in 56 cases,included 36 cases in stage I,14 cases in stage II,and 6 cases in stage III.There were 53 cases of endometrial cancer were diagnosed by transvaginal color Doppler ultrasound and 55 cases were diagnosed by magnetic resonance imaging.The diagnostic sensitivity of transvaginal color Doppler ultrasound and magnetic resonance imaging for endometrial cancer were 92.9%(52/56)and 98.2%(55/56),respectively,with specificity of 97.1%(33/34)and 100.0%(34/34).Transvaginal color Doppler ultrasound diagnosed 32 cases of endometrial cancer in stage I,15 cases in stage II,and 6 cases in stage III.Magnetic resonance imaging diagnosed 35 cases of endometrial cancer in stage I,14 cases in stage II,and 6 cases in stage III.The diagnostic accuracy of transvaginal color Doppler ultrasound and magnetic resonance imaging for clinical staging of endometrial cancer were 95.6%(86/90)and 98.9%(89/90),respectively.The ROC curve shows that the area under the diagnostic curve of transvaginal color Doppler ultrasound and magnetic resonance imaging in patients with endometrial cancer is 0.858 and 0.929,respective
