摘要
目的探讨经阴道超声造影技术联合MRI动态增强扫描对子宫内膜癌肌层浸润深度的应用价值。方法收集120例子宫内膜癌患者,分别采用经阴道超声造影技术、MRI动态增强扫描进行检查,以术后病理检查结果为金标准,观察经阴道超声造影技术、MRI动态增强扫描对子宫内膜癌肌层浸润深度的诊断价值,即灵敏度、特异度、准确度、阳性预测值、阴性预测值。结果以手术病理检查结果为金标准,经阴道超声造影技术检查后,检出深肌层浸润有20例;MRI动态增强扫描检查后,检出深肌层浸润有26例。经阴道超声造影技术、MRI动态增强扫描在灵敏度、阴性预测值方面比较,差异均无统计学意义(P>0.05);MRI动态增强扫描特异度96.71%、准确度95.00%、阳性预测值92.86%,均显著高于经阴道超声造影技术(χ^(2)=4.952,P=0.026;χ^(2)=8.454,P=0.004;χ^(2)=6.879,P=0.009)。结论经阴道超声造影技术、MRI动态增强扫描对子宫内膜癌肌层浸润深度均具有较高的诊断价值,但MRI动态增强扫描准确度、特异度及阳性预测值更高。
Objective To investigate the application value of transvaginal contrast-enhanced ultrasonography combined with MRI dynamic contrast-enhanced scanning in the depth of myometrial invasion in endometrial carcinoma.Methods A total of 120 patients with endometrial cancer who were treated were collected.Transvaginal contrast-enhanced ultrasonography and MRI dynamic enhanced scanning were used for examination.The postoperative pathological examination results were used as the gold standard.To observe the diagnostic value of transvaginal contrast-enhanced ultrasonography and dynamic contrast-enhanced MRI for the depth of myometrial invasion of endometrial carcinoma,namely sensitivity,specificity,accuracy,positive predictive value and negative predictive value.Results Taking the results of surgical and pathological examination as the gold standard,20 cases of deep myometrial infiltration were detected by transvaginal contrast-enhanced ultrasonography,and 26 cases of deep myometrial infiltration were detected by dynamic contrast-enhanced MRI.There was no significant difference in sensitivity and negative predictive value between transvaginal contrast-enhanced ultrasonography and MRI dynamic contrast-enhanced scanning(P>0.05).were significantly higher than those of transvaginal contrast-enhanced ultrasound(χ^(2)=4.952,P=0.026;χ^(2)=8.454,P=0.004;χ^(2)=6.879,P=0.009).Conclusion Transvaginal contrast-enhanced ultrasonography and MRI dynamic contrast-enhanced scanning have higher diagnostic value for the depth of myometrial invasion in endometrial cancer,but the accuracy and specificity of MRI dynamic contrast-enhanced scanning have higher positive predictive value.
