摘要
医方心理契约是医务人员对基于自身在医疗机构中所扮演的角色而产生的关于自身权利与义务的内隐期望,包含了对医疗机构及其政府主管部门的期待和其工作对象、即患方的期待。在住院病房中,医方信任的建立与分化通过心理契约的履行感知与其内隐期望进行不断的对比而实现。如果医方知觉到感知与期望的不匹配,就会引发失望、愤怒、倦怠等负面体验,导致医方信任受损甚至中断。反之,患方的尊重与认可会强化医方的职业认同,缓冲其因工作强度和管理压力带来的不满,减少职业倦怠的发生。医方信任的产生以医方心理契约的全方位满足为前提,而医方心理契约的达成既要求在人际层面得到满足,也需要在制度层面得到保障。
The psychological contract of medical workers is the implicit expectation of medical person-nel on their rights and obligations based on their roles in the medical institution,including their expec-tations of the medical institution and government departments,and expectations of patients and their deputies.In the inpatient ward,the establishment and differentiation of the medical workers'trust in patients are achieved through the constant comparison of the fulfillment perception of the psychological contract and their implicit expectations.If doctors or nurses perceive a mismatch between their percep-tions and expectations,it will trigger negative emotions such as disappointment,anger,burnout,etc.,resulting in damage or even interruption of trust in medical staff.On the contrary,respect and recogni-tion from patients can strengthen the professional identity of medical staff,buffer their dissatisfaction caused by work intensity and management pressure,and reduce the occurrence of job burnout.The formation of medical trust is premised on the all-around satisfaction of the psychological contract of medical workers,which requires not only satisfaction at the interpersonal level,but also insurance at the institutional level.
