摘要
目的探讨脑梗死患者复发的影响因素。方法选取2017年5月至2019年5月于长江大学附属仙桃市第一人民医院接受治疗的116例脑梗死患者作为研究对象,随访1年,根据是否复发将患者分为未复发组(81例)和复发组(35例)。比较两组患者的临床资料,采用多因素logistic回归分析脑梗死复发的影响因素。结果复发组患者的血清超敏C反应蛋白(hs-CRP)、同型半胱氨酸(Hcy)和脂蛋白相关磷脂酶A2(Lp-PLA2)水平高于未复发组,差异有统计学意义(P<0.05);两组患者的性别、年龄、高血压、糖尿病、吸烟、饮酒、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白(LDL)和高密度脂蛋白(HDL)比较,差异无统计学意义(P>0.05)。多因素logistic回归分析结果显示,血清Hcy(OR=1.762,95%CI:1.179~2.633)、Lp-PLA2(OR=2.809,95%CI:1.604~4.920)是影响脑梗死复发的独立危险因素(P<0.05)。结论血清Hcy、Lp-PLA2水平与脑梗死患者复发密切相关,二者水平升高是影响脑梗死复发的独立危险因素。
Objective To explore the influencing factors of recurrence in patients with cerebral infarction.Methods A total of 116 patients with cerebral infarction who were treated in Xiantao First People's Hospital Affiliated to Changjiang University from May 2017 to May 2019 were selected as the study subjects,followed up for 1 year,the patients were divided into a non-recurrence group(81 cases)and a recurrence group(35 cases)according to whether they had a recurrence.The clinical data of two groups of patients were compared.Multiple logistic regression was used to analyze the influencing factors of cerebral infarction recurrence.Results The levels of serum hypersensitive C-reactive protein(hs-CRP),homocysteine(Hcy)and lipoprotein-associated phospholipase A2(Lp-PLA2)in the recurrence group were higher than those in the non-recurrence group,the differences were statistically significant(P<0.05).There were no significant differences in gender,age,hypertension,diabetes,smoking,drinking,total cholesterol(TC),triglyceride(TG),low density lipoprotein(LDL)and high density lipoprotein(HDL)between the two groups(P>0.05).The results of multivariate logistic regression analysis showed that serum Hcy(OR=1.762,95%CI:1.179-2.633)and Lp-PLA2(OR=2.809,95%CI:1.604-4.920)were independent risk factors affecting the recurrence of cerebral infarction(P<0.05).Conclusion The levels of serum Hcy and Lp-PLA2 are closely related to the recurrence of cerebral infarction,and their elevated levels are independent risk factors affecting the recurrence of cerebral infarction.
