血清心肌肌钙蛋白Ⅰ与肌酸激酶同工酶联合检测在病毒性心肌炎诊断中的应用

摘要 目的探讨血清心肌肌钙蛋白Ⅰ(cTnⅠ)与肌酸激酶同工酶(CK-MB)联合检测在病毒性心肌炎诊断中的应用。方法选取2019年1月至2020年1月本院收治的60例心肌炎患者作为研究对象,患者均行血清cTnⅠ、CK-MB检测,以心内膜心肌活检术检查结果作为金标准,分析血清cTnⅠ与CK-MB联合检测在病毒性心肌炎诊断中的应用价值。结果经心内膜心肌活检术检查发现,60例心肌炎患者中病毒性心肌炎41例,细菌性心肌炎11例,寄生虫性心肌炎8例。病毒性心肌炎与非病毒性心肌炎患者性别、年龄比较差异无统计学意义,病毒性心肌炎患者血清cTnⅠ及CK-MB水平均高于非病毒性心肌炎患者,差异有统计学意义(P<0.05)。血清cTnⅠ、CK-MB水平高为病毒性心肌炎发生的危险因素(P<0.05)。绘制ROC曲线,血清cTnⅠ、CK-MB水平检测及联合检测预测病毒性心肌炎的AUC分别为0.746、0.743及0.775,均有一定预测价值,且联合预测效能最好。结论血清cTnⅠ与CK-MB联合检测预测病毒性心肌炎的价值较高,可用于病毒性心肌炎的早期诊断,以指导临床治疗。 Objective To investigate the combined detection of serum cardiac troponinⅠ(cTnⅠ)and creatine kinase isoenzyme(CK-MB)in the diagnosis of viral myocarditis.Methods A total of 60 patients with myocarditis admitted to our hospital from January 2019 to January 2020 were selected as research objects.All patients underwent serum cTnⅠand CK-MB detection.The results of endocardial myocardial biopsy were used as the gold standard to analyze the application value of combined detection of serum cTnⅠand CK-MB in the diagnosis of viral myocarditis were analyzed.Results There were 41 cases of viral myocarditis,11 cases of bacterial myocarditis and 8 cases of parasitic myocarditis among 60 cases of myocarditis.There was no statistical difference in gender and age between the patients with viral myocarditis and non-viral myocarditis,but the serum cTnⅠand CK-MB in patients with viral myocarditis levels were higher than those of non-viral myocarditis,and the differences were statistically significant(P<0.05).High levels of serum cTnⅠand CK-MB were risk factors for viral myocarditis(P<0.05).ROC curve was drawn,and the AUC of serum cTnⅠand CK-MB levels and combined detection were 0.746,0.743 and 0.775,respectively,which had certain predictive value and combined prediction efficiency was the best.Conclusion The combined detection of serum cTnIⅠand CK-MB has high value in predicting viral myocarditis,and can be used for the early diagnosis of viral myocarditis,so as to guide the early clinical treatment.
