摘要
目的探讨MRI与CT检查在腰椎间盘突出诊断中的应用价值。方法选取2017年9月至2020年9月本院收治的72例腰椎间盘突出患者作为研究对象,所有患者均进行CT和MRI检查,以手术诊断结果为金标准,比较两种检查方式不同影像学征象的诊断准确率及阳性率、特异度、灵敏度。结果MRI检查钙化、椎间盘积气的诊断准确率均低于CT检查,椎间盘变形、脊髓变形、硬膜囊受压的诊断准确率均高于CT检查,差异有统计学意义(P<0.05);两种检查方式神经根受压诊断准确率比较差异无统计学意义。MRI检查诊断阳性率、特异度、灵敏度均明显高于CTI检查,差异有统计学意义(P<0.05)。结论MRI与CT检查均有独特的优势,但MRI诊断腰椎盘突出的准确率高于CT检查,临床应根据患者实际情况选择诊断方式,以提高患者临床诊断效能,更好地指导临床治疗。
Objective To explore the application value of MRI and CT in the diagnosis of lumbar disc herniation.Methods 72 patients with lumbar disc herniation admitted to our hospital from September 2017 to September 2020 were selected as the study subjects,and they underwent CT and MRI examinations,taking the surgical diagnosis results as the gold standard,the diagnostic accuracy of different imaging signs,and positive rate,specificity and sensitivity were compared between the two examination methods.Results The diagnostic accuracy of calcification and interverte-bral disc pneumatosis of MRI examination was lower than those of CT examination,and the diagnostic accuracy of intervertebral disc deformation,spinal cord deformation and dural sac compression were higher than those of CT examination,the differences were statistically significant(P<0.05);there was no statistically significant difference in the diagnostic accuracy of nerve root compression between the two examination methods.The diag-nostic positive rate,specificity and sensitivity of MRI examination were significantly higher than those of CT examination,the differences were sta-tistically significant(P<0.05).Conclusion MRI and CT have unique advantages,but the accuracy of MRI in diagnosing lumbar disc herniation is higher than that of CT,the clinical diagnosis method should be selected according to the actual situation of patients to improve the clinical diagnosis efficiency of patients and better guide clinical treatment.
