摘要
目的:探讨T1WI挤压征及肿瘤边缘强化(PRS征)对单发脑转移瘤的诊断价值。方法:选取经临床或手术病理确诊的单发脑转移瘤(SBM)56例,高级别胶质瘤(HGG)20例,均行常规MRI平扫及增强扫描。回顾性分析SBM及HGG的边缘征象,并计算其敏感性和特异性。结果:76例中,45例出现T1WI挤压征,其中41例为SBM,4例为HGG,挤压征诊断SBM的敏感性为73.21%,特异性80%;43例出现PRS征,其中38例为SBM,5例为HGG,PRS征诊断SBM的敏感性为67.85%,特异性为75%,差异均有统计学意义(χ^(2)=17.28、11.02,P<0.05)。29例SBM及2例HGG同时见挤压征和PRS征,SBM出现其中一种或以上征象的几率为89.28%。结论:挤压征及PRS征是SBM较特异的MRI征象,其敏感性和特异性较高,对SBM的诊断有一定的价值,当肿瘤同时出现这两种征象时,高度提示SBM可能。
Objective:To investigate the diagnostic value of TIWI crush sign and peripheral rim sign(PRS sign)in solitary brain metastases.Methods:56 cases of solitary brain metastases(SBM)and 20 cases of high-grade glioma(HGG)confirmed by clinical or surgical pathology were selected and received conventional MRI plain scan and enhanced scan.The marginal signs of SBM and HGG were retrospectively analyzed,and their sensitivity and specificity were calculated.Results:Among 76 cases,45 cass had TIWI crush sign,including 41 cases of SBM and 4 cases of HGG.The sensitivity and specificity of crush sign in the diagnosis of SBM was 73.21%and 80%.43 patients showed PRS sign,including 38 cases of SBM and 5 cases of HGG.The sensitivity and specificity of PRS sign in the diagnosis of SBM were 67.85%and 75%,respectively,with statistically significant differences(χ^(2)=17.28,11.02,P<0.05).29 cases of SBM and2 cases of HGG showed crush sign and PRS sign simultaneously,and the probability of one or more of them was 89.28%.Conclusion:Crush sign and PRS sign are specific MRI signs of SBM,which have high sensitivity and specificity,and are of certain value in the diagnosis of SBM.When these two signs appear at the same time,it is highly suggestive of the possibility of SBM.
