脑转移瘤增强MR表现的回顾性分析

摘要 目的提高脑转移瘤的诊断准确性。方法回顾性分析35例脑转移瘤的MR表现。35例均经1.5T高场强磁共振扫描仪检查,采用T1加权、T2加权和FLAIR序列进行扫描,所选患者平扫后均行Gd-DTPA增强扫描。结果病灶发生于大脑半球19例,小脑6例,全脑8例,脑室内及脑干各1例。病灶多数呈类圆形不均匀长T1、长T2信号灶,发生坏死囊变13例,出血5例。病灶表现为不同程度的占位效应和瘤周水肿;增强后呈环状强化17例、结节状强化12例、花边状强化6例。结论脑转移瘤的MR表现形式多样化,但多数患者有一定特征性,MR增强检查有利于显示转移瘤的特征,对定位定性及定量诊断具有明显的优势。 Objective To improve the diagnostic accuracy of intracerebral metastases tumors in 35 cases.Methods The 35 cases with this tumor were examined with a 1.5T MR system with SE sequence (T1 weighted,T2 wighted) and FLAIR sequence.All the selected cases were examined with contrast medium,Gd-DTPA,after plain scanning.Results The lesions located in cerebral hemispheres,cerebellar hemi-spheres,globe brain,ventricle and brain stem were 19,6,8,1 and 1,respectively. Generally,the lesions were roud-like and heterogeneous intensity in T1 weighted images and T2 weighted images.In the lesions there were necrosis and cyst in 13 cases and bleeding in 5 cases.The lesions were accompanied with mass effect and surrounding edema of tumor to some extent;rim,nodular and lace-like enhancement was found in 17,12 and 6 cases,respectively.Conclusion The MR appearance of intracerebral matastases tumors is diversed,but most of them have a certain specific feature,and the contrast-enhanced MR contributes to showing the feature and has an obvious diagnostic advantage to show their location, character and quantity.
