摘要
目的 研究肺癌脑转移瘤瘤周水肿缺乏的影响因素.方法 回顾性分析17例肺癌脑转移瘤的完整的MRI资料.将转移灶按照有无瘤周水肿分为水肿组和水肿缺乏组.从断层像上观察瘤灶在脑内的分布情况,同时记录转移灶的组织学类型、患者的年龄和性别.对瘤灶的大小、瘤灶中心到最近灰质的距离、瘤灶的强化特点3个变量进行单因素分析.结果 17例肺癌脑转移瘤病人中,男15例,女2例,年龄34~75岁,平均55.71岁.肺癌组织学类型为小细胞肺癌8例,非小细胞肺癌9例;共发现79个瘤灶.瘤周水肿组共46个瘤灶,瘤周水肿缺乏组共33个瘤灶.水肿缺乏组瘤径小于水肿组 (t=3.740,P<0.01),水肿缺乏组瘤灶中心到最近灰质距离小于水肿组(t=1.9962,P<0.01).强化特点方面,两组比较无显著性差异(t=3.065,P>0.05).结论 肺癌脑转移瘤瘤周水肿缺乏与瘤灶大小、瘤灶中心到最近灰质距离有关,与肿瘤强化类型无关.
Objective To study the influential factors of metastasis foci without surrounded edema in brain metastasis from lung cancer.Methods Seventeen cases with pathologically and clinically proved brain metastases from lung cancer were retrospectively reviewed.Emphasis was placed on the number of metastasis foci,diameter of tumor,the distance from tumor center to nearest cortex wall and enhanced features,and the sex,age and histological type were noted.Three variables(diameter of tumor,the distance from tumor center to nearest cortex wall,enhanced features)were investigated by using single factor analysis.Results Fifteen cases were male,2 cases were female.The age ranged from 34 to 75 years old(mean,55.71).Eight cases were small cell lung cancer,9 cases were nonsmall cell lung cancer.Seventy-nine metastasis foci were found in 17 cases.Forty-six foci occurred in the group with surrounded edema,while thirty-three foci occurred in the group without surrounded edema.The average diameter of focus and the distance from tumor center to nearest cortex wall in the group without edema were smaller than that in the group with surrounded edema(P<0.01),whereas the influential factors of metastasis foci without surrounded edema in brain metastasis from lung cancer were no significant difference in enhanced features(P>0.05).Conclusion The influential factors of metastasis foci without surrounded edema in brain metastasis from lung cancer are diameter of focus and the distance from tumor center to nearest cortex wall,but not correlate with enhanced features.
