感觉统合训练改善脑损伤患儿吞咽功能的临床评价 被引量:2

Clinical evaluation of sensory integration training to improve swallowing function in children with brain injury

摘要 目的探讨感觉统合训练联合个性化康复治疗方案对脑损伤鼻饲患儿吞咽功能的改善效果。方法将2021-06—2022-06在武汉大学中南医院接受住院治疗的40例脑损伤鼻饲患儿随机分为对照组(n=20)及观察组(n=20)。对照组采取个性化康复治疗方案,观察组在对照组的基础上采用感觉统合训练。比较2组患儿干预8周后的吞咽功能恢复情况及不良反应发生情况。结果干预8周后,观察组洼田饮水试验评级显著优于对照组(P<0.05)。观察组治疗总有效率(95%)显著高于对照组(65%,P<0.05)。观察组治疗后的视频透视吞咽功能检查(VFSS)评分为(8.73±2.14)分,高于对照组的(7.15±2.26)分,差异有统计学意义(P<0.05)。治疗过程中观察组共发生吸入性肺炎1例,对照组6例,观察组不良反应发生率显著低于对照组(P<0.05)。结论感觉统合训练联合个性化康复治疗方案可有效促进脑损伤鼻饲患儿吞咽功能的恢复,减少不良反应的发生。 Objective To explore effects of sensory integration training combined with personalized reha⁃bilitation therapy on recovery of the swallowing function in nasal-feeding children with brain injury.Methods A total of 40 children with brain injury receiving nasal feeding in Zhongnan Hospital of Wuhan University from June 2021 to June 2022 were randomly divided into control group(n=20)and observation group(n=20).The control group received personalized rehabilitation therapy,and the observation group received sensory integration training on the basis of the control group.The recovery of the swallowing function and the occurrence of adverse reaction were compared between the two groups after 8 weeks of intervention.Results After 8 weeks of intervention,the water swallow test rating of observation group was notably better than that of control group(P<0.05).The total ef⁃fective rate of observation group(95%)was markedly higher than that of control group(65%)(P<0.05).The video fluoroscopic swallow study(VFSS)score of the observation group was(8.73±2.14)points,and was markedly higher than that of the control group(7.15±2.26)points(P<0.05).Aspiration pneumonia occurred in 1 case in the observation group and 6 cases in the control group.The incidence of adverse reaction in the observation group was notably lower than that in the control group(P<0.05).Conclusion Sensory integration training combined with personalized rehabilitation therapy could effectively promote the recovery of the swallowing function in nasal-feeding children with brain injury and reduce the occurrence of adverse reaction.
