摘要
目的探索老年脑卒中患者接受心理护理联合康复训练对患者心理状态及吞咽功能障碍影响。方法将2020年10月-2022年10月期间收治的90例老年脑卒中患者作为研究对象,随机分为观察组(n=46例)和对照组(n=44例)。对照组接受常规护理干预,观察组另外增加心理护理联合吞咽功能训练进行干预。4周后比较两组患者干预前后Hamilton焦虑评分、Hamilton抑郁评分、标准吞咽功能评估表评分及营养指标。结果干预后两组血清铁蛋白、血红蛋白及白蛋白含量均较同组干预前升高,且观察组高于对照组,差异均有统计学意义(P<0.05)。干预后两组Hamilton焦虑抑郁评分均较同组干预前降低,且观察组低于对照组,差异均有统计学意义(P<0.05)。干预后两组标准吞咽功能评估表评分较干预前比均降低,且观察组低于对照组,差异均有统计学意义(P<0.05)。结论老年脑卒中吞咽功能障碍患者接受心理护理联合康复训练,可使患者心理状况、吞咽功能、机体营养状况得到明显改善。
Objective To explore the effect of psychological nursing combined with rehabilitation training on the psychological status and swallowing dysfunction of elderly stroke patients.Methods Ninety elderly stroke patients admitted from October 2020 to October 2022 were randomly divided into observation group(n=46 cases)and control group(n=44 cases).The control group received routine nursing intervention,and the observation group received psychological nursing combined with swallowing function training.After 4 weeks,the Hamilton anxiety score,Hamilton depression score,standard swallowing function evaluation table score and nutritional index of the two groups were compared before and after the intervention.Results After the intervention,the contents of serum ferritin,hemoglobin and albumin in the two groups were higher than those in the same group before the intervention,and the observation group was higher than the control group,the difference was statistically significant(P<0.05).After the intervention,the Hamilton anxiety and depression scores of the two groups were lower than those of the same group before the intervention,and the observation group was lower than the control group,the difference was statistically significant(P<0.05).After the intervention,the score of the standard swallowing function evaluation table of the two groups was lower than that before the intervention,and the observation group was lower than that of the control group,the difference was statistically significant(P<0.05).Conclusion Psychological nursing combined with rehabilitation training can significantly improve the psychological status,swallowing function and nutritional status of elderly patients with dysphagia after stroke.
