摘要
目的探讨全髋关节置换术(THA)治疗髋臼骨折术后并发创伤性关节炎(TA)患者的临床效果。方法回顾性分析27例髋臼骨折术后并发TA患者的临床资料,所有患者均行THA治疗。记录患者随访2年内影像学检查结果的变化情况、THA后1年内髋关节功能的恢复情况[髋臼假体外展角、髋臼前倾角、Harris髋关节功能评分(HHS)]和生活质量评定简表(SF-12)评分,以及术后1年内并发症的发生情况。结果27例患者均顺利完成手术,其中25例患者末次随访时影像学检查结果均提示髋臼假体位置良好,1例患者发生髋臼侧假体无菌性松动合并髋臼钢板断裂,1例患者发生髋臼陶瓷内衬碎裂。术后1年,患者的髋臼假体外展角为(40.49±4.07)°,髋臼前倾角为(13.70±4.94)°,伤口均愈合良好,均未出现下肢静脉血栓、关节感染、假体周围骨折、关节脱位等并发症,且患者的HHS,以及SF-12评分中的生理评分和心理评分均高于术前(均P<0.05)。结论THA是髋臼骨折术后并发TA的有效治疗手段,能够有效改善患者的髋关节功能,提高患者的生活质量,安全可靠,值得临床应用推广。
Objective To explore the clinical effect of total hip arthroplasty(THA)for the treatment of concomitant traumatic arthritis(TA)in patients after acetabular fracture surgery.Methods The clinical data of 27 concomitant TA patients after acetabular fracture surgery were retrospectively analyzed,all patients underwent THA for treatment.Changes of imageological examination results within 2 years of follow-up,recovery of hip joint function within 1 year after THA(abduction angle of acetabular prosthesis,acetabular anteversion angle,and Harris Hip Score[HSS]),12-Item Short-Form Health Survey(SF-12)score,and the occurrence of complications within 1 year after surgery were recorded in patients.Results Surgery was successfully completed in 27 patients,therein at the last follow-up,the imageological examination results of 25 patients indicated that acetabular prosthesis was in a favorable position,and 1 patient suffered from aseptic loosening of acetabular prosthesis and concomitant broken acetabular plate,as well as 1 patient suffered from fragmented ceramic lining of acetabulum.One year after surgery,abduction angel of acetabular prosthesis was(40.49±4.07)°,and acetabular anteversion angle was(13.70±4.94)°;furthermore,their wounds healed well,and no patients suffered from lower limb venous thrombosis,joint infection,periprosthesis fracture,dearticulation of joint,and other complications;in addition,the HHS,and physiological score and psychological score of SF-12 score were all higher as compared with before surgery(all P<0.05).Conclusion THA is an effective therapeutic method for concomitant TA after acetabular fracture surgery,which can effectively ameliorate hip joint function of patients,and improve patients′quality of life.It is safe,reliable,and worthy of clinical application and promotion.
