PG-SGA评分对改善胃癌术后化疗者营养状况及不良反应的影响 被引量:3

Effect of PG-SGA Score on Improving Nutritional Status and Adverse Re⁃actions in Patients with Gastric Cancer undergoing Postoperative Chemo⁃therapy

摘要 目的 探讨主观整体营养状况评估量表(Patient Generated Subjective Global Assessment, PG-SGA)评分对改善胃癌术后化疗者营养状况及不良反应的影响。方法 选取2019年4月—2022年10月涿州市医院普外科收治的99例限期胃癌根治术患者为研究对象,按照随机数表法分为观察组和对照组。对照组(49例)患者接受常规饮食指导,观察组(50例)在对照组的基础上辅以PG-SGA评分指导下进行个性化营养干预。比较两组血清蛋白水平、热量摄入量情况及体质指数情况,分析两组人体成分及化疗不良反应发生情况。结果 干预后,观察组血红蛋白、血清白蛋白及前清蛋白值均高于对照组,差异有统计学意义(P<0.05);观察组热量摄入量情况及体质指数高于对照组,差异有统计学意义(P<0.05)。干预后,观察组体脂含量、体蛋白质量、体骨骼肌量及体水分量水平分别为(15.6±3.1)kg、(8.9±1.9)kg、(26.8±4.1)kg、(31.2±3.1)L,均高于对照组,差异有统计学意义(t=6.000、3.869、5.579、21.063,P<0.05),观察组化疗不良反应发生率为2.0%,显著低于对照组的28.6%,差异有统计学意义(χ^(2)=12.972,P<0.05)。结论 应用PG-SGA评分指导下进行个性化营养,对改善胃癌术后化疗患者营养状况有重要的指导意义,可改善化疗不良反应。 Objective To explore the effect of Patient Generated Subjective Global Assessment score on improving nu‐tritional status and adverse reactions in patients with gastric cancer after chemotherapy.Methods From April 2019 to October 2022,99 patients with limited radical gastrectomy for gastric cancer admitted to the Department of General Surgery of Zhuozhou Hospital were selected as the research objects,and they were divided into observation group and control group according to the random number table method.The control group(49 cases)received routine dietary guidance,and the observation group(50 cases)received personalized nutritional intervention under the guidance of PG-SGA score on the basis of the control group.The serum protein level,calorie intake and body mass index were compared between the two groups,and the body composition and adverse reactions of chemotherapy were analyzed.Results After intervention,the values of hemoglobin,serum albumin and prealbumin in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The heat intake and body mass index of the observation group were higher than those of the control group,and the difference was statisti‐cally significant(P<0.05).After intervention,the levels of body fat,body protein,body skeletal muscle and body water content in the observation group were(15.6±3.1)kg,(8.9±1.9)kg,(26.8±4.1)kg and(31.2±3.1)L,respectively,which were higher than those in the control group,the difference was statistically significant(t=6.000,3.869,5.579,21.063,P<0.05).The adverse reaction rate of chemotherapy in the observation group was 2.0%,which was significantly lower than 28.6%in the control group,the difference was statistically significant(χ^(2)=12.972,P<0.05).Conclusion Person‐alized nutrition under the guidance of PG-SGA score has important guiding significance for improving the nutritional status of patients with gastric cancer after chemotherapy and improving the adverse reactions of chemotherap
