摘要
目的:分析血清脂蛋白相关磷脂酶A2(Lp-PLA2)、白介素1β(IL-1β)、肿瘤坏死因子α(TNF-α)水平与冠心病冠状动脉病变支数和狭窄程度的关系,为冠心病的预测和风险评估提供依据。方法:选取2020年6月—12月甘肃省人民医院心内科收治的138例确诊冠心病患者。根据冠脉造影血管狭窄的病变支数分为单支病变、双支病变及三支病变组。根据冠脉血管狭窄程度分为轻度狭窄(50%~75%)和重度狭窄(>75%)。另选取健康体检者30名作为对照组。对研究对象Lp-PLA2、IL-1β及TNF-α水平进行观察和比较,数据采用SPSS 22.0统计软件进行统计分析。结果:血清Lp-PLA2、IL-1β、TNF-α水平三个冠脉病变支数组和两个冠脉狭窄组均高于对照组,差异有统计学意义(P<0.05)。血清Lp-PLA2水平三支病变组>双支病变组>单支病变组,重度狭窄组>轻度狭窄组,差异均有统计学意义(P<0.05)。血清IL-1β水平三支、双支病变组均高于单支病变组(P<0.05),而TNF-α水平三支冠脉病变组高于双支和单支病变组,重度狭窄组高于轻度狭窄组(P<0.05)。相关性分析发现血清Lp-PLA2、TNF-α水平与冠心病冠脉病变支数、冠脉狭窄程度均呈正相关。血清IL-1β水平与冠脉病变支数呈正相关(P<0.05),与冠脉狭窄程度无相关性(P>0.05)。结论:血清Lp-PLA2、IL-1β、TNF-α水平可不同程度的反映冠心病冠状动脉病变程度,可为冠心病的预测和风险评估提供一定的依据。
Objective To analyze the relationship between serum lipoprotein-associated phospholipase A2(Lp-PLA2),interleukin 1 beta(IL-1β),tumor necrosis factor alpha(TNF-α)levels and the number of coronary artery lesions and the degree of stenosis in coronary heart disease(CHD),and to provide evidence for the prediction and risk assessment of CHD.Methods A total of 138 patients with confirmed CHD admitted to the Department of Cardiology of Gansu Provincial Hospital,from June to December 2020 were selected.According to the lesion number of coronary angiography stenosis,they were divided into single vessel stenosis,double vessel stenosis and three vessel stenosis groups.According to the degree of coronary stenosis,they were divided into mild stenosis group(50%~75%)and severe stenosis group(>75%).Another 30 healthy subjects were selected as the control group.The levels of Lp-PLA2,IL-1β and TNF-α in the subjects were observed and compared,and the data using SPSS 22.0 statistical software for statistical analysis.Results The serum levels of Lp-PLA2,IL-1β and TNF-α in three vessel stenosis groups and double vessel stenosis groups were higher than those in the control group,with statistical significance(P<0.05).There were statistically significant differences in serum Lp-PLA2 level,the three vessel lesion group>double vessel lesion group>single vessel lesion group and the severe vessel stenosis group>mild vessel stenosis group(P<0.05).The level of serum IL-1β,the three and double vessel stenosis group were higher than in vessel stenosis group(P<0.05).The serum TNF-α level in the three vessel stenosis group was higher than that in the double vessel stenosis and single vessel stenosis groups,and that in the severe stenosis group was higher than that in the mild stenosis group(P<0.05).Correlation analysis showed that serum Lp-PLA2 and TNF-α levels were positively correlated with the number of coronary artery lesions and the degree of coronary artery stenosis in CHD.Serum IL-1β level was positively correlated with the number
