摘要
卒中后吞咽障碍(PSD)是脑卒中后最常见的并发症。针灸作为治疗中风的补充方法,已经成为PSD治疗的指南和共识,其中风池穴是临床针灸处方的常用穴。在临床应用上,对该穴施以不同的行针手法、配伍不同穴位与结合不同的治疗手段等均能明显改善吞咽困难的症状,起到利咽通窍的作用。在机制研究上,针刺风池穴通过影响神经元的凋亡、改善脑血流量、促进神经修复和发生及增强吞咽肌力等方面促进PSD患者的康复。本研究综述并讨论了从风池穴治疗PSD的理论依据、临床应用、机制研究以及目前存在的不足之处,以期为今后进一步应用于临床和深入研究提供参考。
Post-stroke dysphagia(PSD)is the most common post-stroke complication.Acupuncture,as a complementary treatment for stroke,has become the guideline and consensus for the treatment of PSD,with Fengchi(GB20)being a commonly used point in clinical acupuncture prescriptions.In clinical application,different acupuncture manipulations,different acupoints compatibility and different treatment methods of GB20 can significantly improve the symptoms of dysphagia,and play a role in facilitating swallowing and clearing orifice.In terms of mechanistic studies,needling GB20 can promote the recovery of PSD patients by affecting neuronal apoptosis,improving cerebral blood flow,promoting neural repair and occurrence,and enhancing swallowing muscle strength.This study reviewed and discussed the theoretical basis,clinical application,mechanistic studies and current shortcomings of needling GB20 in treatment of PSD,with a view to providing reference for further clinical application and in-depth studies in the future.
