电针督脉穴对脑梗死恢复期患者神经功能恢复及炎症因子、凝血状态影响时效性临床研究 被引量:3

摘要 目的 探讨电针督脉经穴对脑梗死恢复期患者神经功能恢复及炎症因子和凝血状态影响是否存在时间与疗效的正相关性。方法 选取符合条件的60例脑梗死恢复期患者,按随机数字表随机分为对照组28例,观察组32例,对照组予以常规康复训练,观察组在对照组基础上予以电针百会、风府,分别于治疗前及治疗3、7、14 d后进行神经功能缺损评分评定,采取酶联免疫吸附试验(ELISA)检测患者血清炎症因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)]以及凝血状态指标[血小板分子标志物1(PAC-1)、D-二聚体(D-D)]含量,组内组间进行统计学分析,比较差异性,分析时间与疗效有无正相关性。结果 观察组与对照组患者神经功能评分均有所改善(P<0.05,P<0.01),治疗时间越长,患者神经功能改善效果越佳,呈时间与疗效的正相关性,且观察组总体效果更显著。观察组和对照组患者炎症因子(TNF-α、IL-1β)及凝血状态指标(PAC-1、D-D)含量均有所降低(P<0.05,P<0.01),观察组相关炎症因子和凝血状态指标降低程度更显著(P<0.01);对照组患者TNF-α、IL-1β、PAC-1、D-D含量变化与治疗时间呈正相关性;观察组患者TNF-α、IL-1β、PAC-1、D-D含量改善在治疗7 d后达到高峰(P<0.01),而更长时间的治疗组内比较差异无统计学意义(P>0.05)。该研究发现通过电针督脉经穴治疗脑梗死恢复期患者,其治疗时间与神经功能恢复呈正相关性,而对炎症因子以及高凝状态的改善在治疗7 d内呈正相关性,在治疗7 d后治疗时间与疗效无明显相关性。结论 电针督脉穴可以有效促进脑梗死恢复期患者神经功能恢复、降低炎症因子以及减轻高凝状态,从而能促进功能恢复,提高患者生活质量,临床疗效显著。 Objective To explore whether there is a positive correlation between time and therapeutic effect of electroacupuncture at the acupoints of the Du meridian on the recovery of neurological function,inflammatory factors,and coagulation status in patients with cerebral infarction during the recovery period.Methods A total of 60 patients with convalescent cerebral infarction were selected and randomly divided into the control group(28 cases)and the observation group(32 cases)according to the random number table.The control group received routine rehabilitation training,while the observation group received electroacupuncture at Baihui(DU20)and Fengfu(GV16)on the basis of the control group,Neurological impairment scores were performed before treatment,3 d,7 d and 14 d after treatment,respectively.Enzyme-linked immunosorbent assay(ELISA)was used to detect the contents of inflammatory factors(TNF-α,IL-1β)and coagulation status indicators[platelet molecular marker 1(PAC-1)and D-dimer(D-D)]in serum of the patients.Statistical analysis was performed within and between groups to compare the differences,and whether the analysis time was positively correlated with the therapeutic effect.Results Neurological function scores in both the observation group and the control group were improved(P<0.05,P<0.01).The longer the treatment time,the better effect of neurological function improvement,showing a positive correlation between time and efficacy,and the overall effect of the observation group was more significant.The contents of inflammatory factors(TNF-α,IL-1β),and coagulation status indexes(PAC-1 and D-D)in observation group and control group were decreased(P<0.05,P<0.01),and the reduction degree of related inflammatory factors and coagulation status indexes in observation group was more significant(P<0.01).The contents of TNF-α,IL-1β,PAC-1 and D-D in control group were positively correlated with treatment time.The contents of TNF-α,IL-1β,PAC-1 and D-D in observation group reached the peak after 7 days of treatment(P<0.01
