摘要
目的评估丁苯酞联合舍曲林应用在帕金森病(PD)并抑郁症患者中的效果及对心理情绪、症状改善的影响。方法纳入2015年1月-2020年12月的70例PD并抑郁症患者,参照随机数字表法将其划分为对照组(纳入35例,行舍曲林治疗)、观察组(纳入35例,行丁苯酞联合舍曲林治疗),评价组间治疗有效率、帕金森综合征评定量表(UPDRS)评分、汉密尔顿抑郁量表(HAMD)评分、简易精神状态量表(MMSE)评分、蒙特利尔认知评估量表(MoCA)评分、帕金森病睡眠量表(PDSS)评分、日常生活活动能力量表(ADL)评分、血清学指标。结果①较之对照组(68.57%),观察组治疗有效率(91.43%)更高,P<0.05;②治疗12周后,观察组UPDRS、HAMD、MMSE、MoCA、PDSS、ADL评分和血清学指标[5-羟色胺(5-HT)、脑源性神经营养因子(BDNF)、神经营养因子-3(NT-3)]更佳,P<0.05。结论对PD并抑郁症患者行丁苯酞联合舍曲林治疗,能够提高5-HT、BDNF、NT-3水平,增强疗效,改善PD症状、抑郁情绪、精神状态、认知功能、睡眠状况、日常生活活动能力,值得推广。
Objective To evaluate the effect of butylphthalide combined sertraline in patients with Parkinson's disease(PD)and depression and its influence on the improvement of psychological mood and symptoms.Methods 70 patients with PD and depression from January 2015 to December 2020 were divided into the control group(35 patients were included in the treatment with sertraline)and the observation group(35 patients were included in the treatment with butylphthalide+sertraline)according to the random number table method,and the treatment effectiveness,the Parkinson's syndrome rating scale(UPDRS),the Hamilton depression scale(HAMD),the simple mental state scale(MMSE),the Montreal cognitive assessment scale(MoCA),Parkinson's disease sleep scale(PDSS),activity of daily living(ADL),and serological indicators.Results①Compared with the control group(68.57%),the effective rate of treatment in the observation group(91.43%)was higher,P<0.05;②After 12 weeks of treatment,UPDRS,HAMD,MMSE,MoCA,PDSS,ADL,serological indexes[5-HT,BDNF,NT-3]in the observation group were better,P<0.05.Conclusion The treatment of butylphthalide+sertraline in patients with PD and depression can increase the levels of 5-HT,BDNF and NT-3,enhance the curative effect,improve the symptoms of PD,depressive mood,mental state,cognitive function,sleep status,and activities of daily living,which is worthy of promotion.
