摘要
目的探讨以行为转变理论为指导的示范性教育在前列腺癌患者盆底肌功能锻炼的应用效果。方法选取2020年4月至12月浙江省肿瘤医院收治的84例前列腺癌根治术患者,按随机数字表法将其分为对照组和试验组,每组各42例。对照组患者给予常规宣教,试验组患者进行以行为转变理论为指导的示范性教育干预,比较两组患者的盆底肌功能锻炼依从性、并发症发生率及生活质量。结果干预3个月、6个月,试验组患者的盆底肌功能锻炼依从性及自我效能均优于对照组(P<0.05);干预12个月后,试验组患者的并发症总发生率显著低于对照组(16.67%vs.38.10%,χ^(2)=4.850,P=0.028);干预12个月,试验组患者的躯体功能、角色功能、认知功能、情绪功能、社会功能、疲乏、食欲丧失、失眠、经济困难及总体健康状况评分均显著高于对照组(P<0.05)。结论以行为转变理论为指导的示范性教育可提高前列腺癌根治术患者术后盆底肌锻炼的依从性,加快盆底肌功能恢复,减少并发症发生,改善生活质量。
Objective To explore the effect of demonstration education guided by behavior change theory on pelvic floor muscle function exercise in patients with prostate cancer.Methods A total of 84 patients with radical prostate cancer treated in Zhejiang Cancer Hospital from April to December 2020 were selected and divided into control group and experimental group according to random number table method,with 42 cases in each group.Patients in control group were given routine education,and patients in experimental group were given demonstrative education intervention guided by behavior change theory.The compliance of pelvic floor muscle function exercise,incidence of complications and quality of life of the two groups were compared.Results After 3 months and 6 months of intervention,the compliance of pelvic floor muscle function exercise and self-efficacy in experimental group were better than those in control group(P<0.05).After 12 months of intervention,the total incidence of complications in experimental group was significantly lower than that in control group(16.67%vs.38.10%,χ^(2)=4.850,P=0.028).After 12 months of intervention,the scores of physical function,role function,cognitive function,emotional function,social function,fatigue,loss of appetite,insomnia,economic difficulties and general health status of experimental group were significantly higher than those of control group(P<0.05).Conclusion The demonstration education guided by behavior change theory can improve the compliance of pelvic floor muscle exercise after radical prostatectomy,accelerate the recovery of pelvic floor muscle function,reduce complications and improve the quality of life.
