摘要
目的:探讨显微镜辅助下颈椎前路精细化减压治疗脊髓型颈椎病(CSM)的临床效果。方法:回顾性分析2019年8月—2021年9月行颈前手术治疗的60例CSM患者的临床资料,依据手术方式不同分为开放组30例(行常规颈前路开放手术)及显微组30例(行显微镜辅助下精细化减压内固定术)。对比两组围术期指标、颈椎功能评分、疼痛评分、颈椎功能障碍指数量表(NDI)、生活质量评分及并发症发生率。结果:开放组住院时间长于显微组,术中出血量及术后引流量均大于显微组,差异有统计学意义(P<0.05);两组手术时间及引流管留置时间比较,差异无统计学意义(P>0.05);术后,开放组日本骨科学会(JOA)评分、NDI评分及生活质量各项评分均低于显微组,视觉模拟评分(VAS评分)高于显微组,差异有统计学意义(P<0.05)。开放组并发症发生率高于显微组,差异有统计学意义(P<0.05)。结论:CSM患者应用显微镜辅助下颈椎前路精细化减压治疗能够有效改善颈椎功能,减轻患处疼痛感,提升患者的生活质量,并发症少,安全可靠。
Objective:To investigate the clinical effect of microscopically assisted anterior cervical fine decompression in the treatment of cervical spondylotic myelopathy(CSM).Methods:The clinical data of 60 patients with CSM who underwent anterior cervical surgery in hospital from August 2019 to September 2021 were analyzed retrospectively.According to the different surgical methods,30 patients were divided into open group(routine anterior cervical open surgery)and 30 patients in microscope group(microscopically assisted fine decompression and internal fixation).Perioperative indexes,cervical function score and pain score,cervical dysfunction index(NDI),quality of life score and complication rate were compared between the two groups.Results:The hospitalization time of the open group was longer than that of the microscopic group,and the intraoperative bleeding volume and postoperative drainage volume were higher than those of the microscopic group,with statistical difference(P<0.05).There was no statistical difference between the two groups in operation time and indwelling time of drainage tube(P>0.05).After operation,the scores of japanese orthopaedic association(JOA),NDI and quality of life in the open group were lower than those in the microscopic group,and the VAS score was higher than that in the microscopic group,with statistical difference(P<0.05).The incidence of complications in the open group was higher than that in the microscopic group,there was statistical difference between the two groups(P<0.05).Conclusion:The microscopically assisted anterior cervical fine decompression treatment for CSM patients can effectively improve the cervical function,relieve the pain in the affected area,and improve the quality of life of patients.It is safe and reliable with few complications.
