摘要
【目的】观察膈下逐瘀汤治疗胸椎椎体成形术(PVP)后残余肋间神经痛的临床疗效。【方法】将32例胸椎PVP术后仍残余肋间神经痛的患者随机分为观察组和对照组,每组各16例。对照组给予口服塞来昔布胶囊治疗,观察组在对照组的基础上联合膈下逐瘀汤内服治疗,疗程为4周。观察2组患者治疗前及治疗3 d、2周、4周后疼痛视觉模拟量表(VAS)评分及Oswestry功能障碍指数(ODI)评分的变化情况,并评价2组患者的临床疗效。【结果】(1)临床疗效方面,治疗4周后,观察组的总有效率为93.75%(15/16),对照组为81.25%(13/16),组间比较(秩和检验),观察组的疗效明显优于对照组(P<0.05)。(2)VAS评分方面,治疗后,除对照组治疗3 d后的VAS评分改善不明显(P>0.05)外,2组患者各个观察时间点的VAS评分均较治疗前明显改善(P<0.05),且观察组在治疗2周和4周后对VAS评分的改善作用均明显优于对照组(P<0.05)。(2)ODI评分方面,治疗3 d、2周、4周后,2组患者的ODI评分均较治疗前改善(P<0.05),且观察组在治疗2周和4周后对ODI评分的改善作用均明显优于对照组(P<0.01)。【结论】在常规西药治疗基础上联合膈下逐瘀汤在改善胸椎PVP术后残余肋间神经痛方面疗效确切,其疗效优于单纯常规西药治疗。
Objective To observe the clinical efficacy of Gexia Zhuyu Decoction in the treatment of residual intercostal neuralgia after thoracic percutaneous vertebroplasty(PVP).Methods Thirty-two patients with residual intercostal neuralgia after thoracic PVP were randomly divided into an observation group and a control group,with 16 patients in each group.The control group was given oral use of Celecoxib Capsules,while the observation group was treated with Gexia Zhuyu Decoction and Celecoxib Capsules orally.The course of treatment for the two groups covered 4 weeks.The changes of pain visual analogue scale(VAS)scores and Oswestry dysfunction index(ODI)scores in the two groups were observed before treatment and after 3 days,2 weeks and 4 weeks of treatment.Furthermore,the clinical efficacy of the two groups was evaluated.Results(1)After 4 weeks of treatment,the overall effective rate of the observation group was 93.75%(15/16)and that of the control group was 81.25%(13/16),and the intergroup comparison(tested by rank sum test)showed that the clinical efficacy of the observation group was significantly superior to that of the control group(P<0.05).(2)After treatment,the VAS scores of the two groups at each observation time point were significantly improved compared with those before treatment(P<0.05)except for the VAS scores in the control group after 3 days of treatment(P>0.05).The intergroup comparison showed that the improvement of VAS scores after 2 and 4 weeks of treatment in the observation group was significantly superior to that in the control group(P<0.05).(3)After 3 days,2 weeks and 4 weeks of treatment,the ODI scores of patients in both groups were improved compared with those before treatment(P<0.05),and the improvement of ODI scores after 2 and 4 weeks of treatment in the observation group was significantly superior to that in the control group(P<0.01).Conclusion In addition to conventional western medicine,the combination of Gexia Zhuyu Decoction exerts certain effect in relieving residual intercostal neuralgi
