摘要
目的对宁波市奉化区引起食源性腹泻病例主动监测,了解食源性疾病发病趋势及病原分布变化情况。方法按照《国家食源性疾病监测工作手册》《浙江省食源性疾病监测工作手册》对粪便标本进行沙门菌、志贺菌、副溶血性弧菌、致泻性大肠埃希菌监测。结果2019年—2021年致病菌阳性率分别为7.90%、10.67%、7.78%,差异有统计学意义(P<0.05);3年均以沙门菌检出病例数最多,副溶血性弧菌检出病例数次之;不同性别阳性率比较差异无统计学意义(χ^(2)=0.01,P>0.05);0岁~10岁食源性腹泻病例监测阳性率最高,为39.24%,30岁~40岁次之,不同年龄阳性率比较差异有统计学意义(χ^(2)=202.99,P<0.05);以第三季度(7月—9月)阳性率最高,达到11.69%,第四季度(10月—12月)次之,不同季节阳性率比较差异有统计学意义(χ^(2)=34.94,P<0.05)。结论2019年—2021年宁波市奉化区食源性腹泻病例,主要是由沙门菌、副溶血性弧菌引起,因此要加强食源性腹泻病例的主动监测,及时了解致病菌病原谱的动态变化,防止由食源性引起的疾病的蔓延和暴发。
Objective The purpose of this study is to actively monitor foodborne diarrhea in Fenghua District of Ningbo City,so as to understand the trend of foodborne diseases and the change of pathogen distribution.Methods Stool samples were collected for the detection of Salmonella,Shigella,Vibrio parahaemolyticus,and diarrheogenic Escherichia coli in accordance with the National Foodborne Disease Surveillance Work Manual and Zhejiang Province Foodborne Disease Surveillance Work Man⁃ual.Results The positive rate of pathogenic bacteria detected from 2019 to 2021 was 7.90%,10.67%,and 7.78%,respec⁃tively,and the difference was statistically significant(P<0.05).Salmonella was detected the most in 2019-2021,followed by Vibrio parahaemolyticus.There was no statistical significance on the difference in the positive rate between genders(χ^(2)=0.01,P>0.05).The positive rate was the highest in the age group of 0-10 years old with the proportion of 39.24%,followed by that in the group 30-40 years.There was statistical significance on the differences among different ages(χ^(2)=202.99,P<0.05).The highest positive rate was 11.69%in the third season(July-September),followed by that in the fourth season(October-December),and there was statistical significance on the differences among different seasons(χ^(2)=34.94,P<0.05).Conclusion Foodborne diarrhea cases in Fenghua District from 2019 to 2021 were mainly caused by Salmonella and Vibrio parahaemolyticus.Therefore,active surveillance on foodborne diarrhea should be strengthened,and the dynamic change of pathogen spectrum should be understood timely to prevent the spread and outbreak of foodborne diseases.
