摘要
目的本研究旨在探讨不同微创颅内血肿清除术对老年基底节区高血压性脑出血患者神经功能恢复的影响。方法回顾性收集2020年1月至2021年12月我院收治的老年基底节区高血压性脑出血患者260例,根据患者接受的手术方式,将患者分为观察组120例(立体定向辅助下神经内镜手术血肿清除术),对照组140例(立体定向血肿清除术),比较2组患者术后康复情况。结果观察组与对照组患者年龄、性别、脑出血量、破入脑室、格拉斯哥昏迷评分、高血压分级、高脂血症、糖尿病以及手术时间比较,差异无统计学意义(P>0.05)。观察组与对照组术前美国国立卫生研究院卒中量表(NIHSS)评分比较无显著差异[(22.61±4.92)分vs(23.71±5.32)分,P=0.087)]。与对照组比较,观察组术后3个月、6个月NIHSS评分显著降低[(5.92±4.03)分vs(9.22±3.46)分,P=0.000;(4.51±2.52)分vs(6.51±3.05)分,P=0.000]。2组患者格拉斯哥预后评分和术后并发症比较无统计学差异(P>0.05)。结论立体定向辅助下神经内镜手术血肿清除术可改善老年基底节区高血压性脑出血患者的术后神经功能。
Objective To investigate the effects of different minimally invasive intracranial hematoma removal operations on the neurological recovery of elderly patients with hypertensive intra-cerebral hemorrhage in the basal ganglia region.Methods Clinical data 260 patients with hypertensive intracerebral hemorrhage in the basal ganglia region admitted in our hospital from January 2020 to December 2021 were retrospectively collected.According to the surgical treatments,they were divided into the observation group(stereotactic assisted endoscopic hematoma evacuation,n=120)and the control group(stereotactic hematoma removal,n=140).The outcomes were compared between the two groups.Results There were no significant differences in age,sex,amount of cerebral hemorrhage,ventricular rupture,Glasgow coma scale score,hypertension grade,hyperlipidemia,diabetes and operation time between the two groups(P>0.05),or in NIHSS score before operation(22.61±4.92 vs 23.71±5.32,P=0.087).Compared with the control group,the NIHSS score was decreased significantly in the observation group in 3(5.92±4.03 vs 9.22±3.46,P=0.000)and 6 months after operation(4.51±2.52 vs 6.51±3.05,P=0.000).No statistical differences were observed in Glasgow outcome scale score and postoperative complications between the two groups(P>0.05).Conclusion Stereotactic assisted endoscopic hematoma removal can improve the neurological function of elderly patients with hypertensive intracerebral hemorrhage in the basal ganglia region.
