IVIM-DWI和T1 mapping动态评估COVID-19康复者肝脏损伤

Dynamic Evaluation of Liver Injury in Patients Recovered from COVID-19 via IVIM-DWI and T1 Mapping

摘要 目的探讨体素内不相干运动扩散加权成像(IVIM-DWI)与纵向弛豫时间定量成像(T1 mapping)动态评估COVID-19康复者肝脏损伤的价值。资料与方法前瞻性纳入2020年1—3月阜阳市第二人民医院出院的24例COVID-19康复者(重症型5例,普通型19例)和22名年龄匹配的非COVID-19志愿者(对照组),分别于出院后3个月和1年进行1次腹部MRI随访扫描(包括MRI IVIM-DWI和T1 mapping),纳入3个月随访组和1年随访组(各24例)。比较不同组间肝脏影像学参数的差异,包括表观扩散系数[(ADC)、真性扩散系数(D)假性扩散系数(D^(*))、灌注分数(f)、f×D^(*)值、T1弛豫时间]。结果3个月随访时,重症型、普通型组肝脏T1弛豫时间明显低于对照组(H=8.883,P=0.003),肝脏ADC(H=6.969,P=0.031)、D值显著高于对照组(H=6.224,P=0.045);与普通型组相比,重症型患者肝脏D^(*)、f×D^(*)值显著升高(P均<0.01)。与3个月随访组相比,1年随访组的肝脏D值明显下降(P=0.001)。结论IVIM-DWI和T1 mapping检测到的肝脏影像参数动态变化提示COVID-19康复者发展为轻度、无症状的肝脏损伤,可能随着时间推移而缓解。 Purpose To explore the application value of intravoxel incoherent motion-diffusion weighted imaging(IVIM-DWI)and T1 mapping in dynamic monitoring of liver function damage in patients recovered from COVID-19.Materials and Methods This prospective study included 24 discharged COVID-19 participants(5 patients with severe type and 19 patients with common type)and 22 age-matched non COVID-19 volunteers from January to March 2020 in No.2 People's Hospital of Fuyang City who underwent abdominal MRI with IVIMDWI and T1 mapping sequencing as a COVID-19 group and control group.A total of 24 patients underwent two follow-up MRI scans,and were enrolled as the 3-month follow-up group and 1-year follow-up group.Imaging parameters of the liver,including apparent diffusion coefficient(ADC),D,D^(*),f,f×D^(*)and T1 mapping,were measured.All collected values were compared among different groups.Results The T1 value of the liver in the 3-month follow-up group was the lowest,which was significantly lower than that in the control group(H=8.883,P=0.003),the liver ADC(H=6.969,P=0.031)and D value(H=6.224,P=0.045)of 3-month follow-up group were significantly higher than those of the control group.The liver D^(*)and f×D^(*)values of severe type patients were significantly higher than those of common type patients in the 3-month follow-up group(all P<0.01).The liver D value of the 1-year follow-up group was significantly lower than that of 3-month follow-up group(P=0.001).Conclusion The dynamic alterations of hepatic imaging parameters detected by IVIM-DWI and T1 mapping suggest that COVID-19 survivors might develop mild,non-symptomatic liver impairments,probably relieve over time.
