摘要
目的 分析老年2型糖尿病(T2DM)患者感染幽门螺杆菌(Hp)后对其脂代谢、胰岛功能所产生的影响。方法 选取2020年11月-2021年11月本中心收治的老年T2DM患者70例,开展13C呼气试验,汇总Hp感染情况,并进行准确记录;把患者分成2组(依据有无感染Hp进行分组):感染组(31例)与未感染组(39例),对比两组血清学指标、糖代谢和胰岛功能指标、尿液指标与颈动脉内膜中层厚度(IMT);开展多因素logistic回归分析,总结对老年T2DM患者感染Hp造成影响的主要因素。结果 感染组血清TC、TG、LDL-C、FPG、2hPG、HbA1c、FINS、HOMA-IR、ACR均较未感染组高(P<0.05),而HDL-C较未感染组低(P<0.05),颈动脉IMT较未感染组厚(P<0.05)。经多因素logistic回归分析得知,HOMA-IR、HbA1c、FINS均为老年T2DM患者感染Hp的主要影响因素。结论 针对老年T2DM患者,其有着较高的Hp感染率,且感染者与未感染患者相比,有着更重的动脉粥样硬化、胰岛素抵抗及糖脂代谢紊乱情况;而HOMA-IR、Hb A1c、FINS都是诱发老年T2DM患者感染Hp的影响因素。
Objective To analyze effect of Helicobacter pylori(Hp)infection on lipid metabolism and islet function of elderly patients with type 2 diabetes mellitus(T2DM).Methods The paper chose 70 elderly T2DM patients admitted to our center from November 2020 to November 2021 for 13C breath test,summarized Hp infection status,recorded it accurately,and divided patients into two groups(based on presence or absence of Hp infection):infected group(31 cases)and uninfected group(39 cases).Serological indicators,glucose metabolism and pancreatic function indicators,urine indicators,and carotid intima media thickness(IMT)were compared between two groups;main factors affecting Hp infection of elderly T2DM patients were summarized based on multivariate logistic regression analysis.Results Serum TC,TG,LDL-C,FPG,2hPG,HbA1c,FINS,HOMA-IR,ACR of infected group were higher than uninfected group(P<0.05),while HDL-C was lower than uninfected group(P<0.05),and carotid IMT was thicker than uninfected group(P<0.05).Based on multivariate logistic regression analysis,it was found that HOMAIR,HbA1c,and FINS were main influencing factors for Hp infection of elderly T2DM patients.Conclusion Elderly T2DM patients have a higher infection rate of Hp,and more severe atherosclerosis,insulin resistance,and glucose and lipid metabolism disorders than uninfected patients.And HOMA-IR,HbA1c,and FINS are influencing factors that induce Hp infection of elderly T2DM patients.
