摘要
目的探讨针灸联合五行音乐疗法对脑瘫患儿负性情绪、功能状态及预后的改善作用。方法按照1∶1原则和随机数字表将2019年10月至2022年5月平顶山市第一人民医院收治的脑瘫患儿90例分为观察组、对照组,各45例。两组均给予常规治疗,对照组给予五行音乐疗法,在此基础上观察组给予针灸治疗,均治疗6个月。比较两组临床疗效、负性情绪[汉密尔顿抑郁量表(HAMD)评分]、功能状态[粗大运动功能测试量表-88(GMFM-88)评分、肌张力分级量表(Ashworth)评分、功能性Berg平衡量表(BBS)评分]、运动神经机能(F波波幅及刺激阈值)、生活质量[中国康复研究中心儿童日常生活活动能力(ADL)评分]。结果观察组总有效率95.56%高于对照组80.00%(P<0.05);观察组治疗3、6个月后HAMD评分均低于对照组(P<0.05);治疗3、6个月后,观察组GMFM-88、BBS评分高于对照组,Ashworth评分低于对照组(P<0.05);与对照相比,观察组治疗3、6个月后F波波幅下降更为明显,F波刺激阈值上升更为明显(P<0.05);治疗3、6个月后,两组ADL评分均呈持续上升趋势,其中观察组更为明显(P<0.05)。结论针灸联合五行音乐疗法治疗脑瘫,能有效改善患儿粗大运动功能、平衡功能及运动神经机能,缓解负性情绪,减轻痉挛程度,提高生活质量,疗效确切。
Objective To explore the effect of acupuncture combined with five elements music therapy on improving negative emotions,functional status and prognosis of children with cerebral palsy.Methods A total of 90 children with cerebral palsy admitted to the First people's Hospital of Pingdingshan from October 2019 to May 2022 were recruited and randomLy divided into observation group and control group at a ratio of 1∶1,with 45 cases in each group.All recruited children were managed by routine treatment and five elements music therapy for 6 months,and those in observation group were additionally given to acupuncture treatment.The clinical efficacy,negative emotions(Hamilton depression scale[HAMD]score),functional status(gross motor function test scale-88[GMFM-88]score,muscle tension rating scale[Ashworth]score,Functional Berg balance scale[BBS]score),motor nerve function(F wave amplitude and stimulation threshold)and quality of life(Activity of daily living[ADL]score of children in China Rehabilitation Research Center)were compared between groups.Results The total effective rate in the observation group was significantly higher than that in the control group(95.56%VS 80.00%,P<0.05).HAMD scores in the observation group were significantly lower than those in the control group after 3 and 6 months of treatment(P<0.05).After 3 and 6 months of treatment,GMFM-88 and BBS scores in the observation group were significantly higher than those in the control group,while Ashworth scores were significantly lower than those in the control group(P<0.05).Compared with the control group,the amplitude of F wave in the observation group significantly decreased and the stimulation threshold of F wave significantly increased after 3 and 6 months of treatment(P<0.05).After 3 and 6 months of treatment,the ADL scores in the both groups showed a continuous upward trend,which were significantly pronounced in the observation group(P<0.05).Conclusion Acupuncture combined with five elements music therapy can effectively improve the gross motor func
