摘要
多囊卵巢综合征(PCOS)是目前青春期、育龄期女性最常见的内分泌及代谢性疾病之一,具有病因病机复杂、临床表现多样的特点,不仅具有明显的近期临床影响,还是多种高发疾病的高危致病。中医对本病的认识可从多个角度分析,不仅可从脏腑经络学说进行辨证施治,亦可从痰、湿、瘀血等病理产物进行辨证分析,笔者跟随贺师门诊,从脏腑辨证角度结合痰、湿、瘀等病理产物对该病进行简单分析。贺师认为:多囊卵巢综合征根据中医理论进行辨证分型,结合病史、症状、体征等总结得出,痰湿夹瘀证最为常见,且症状较为典型,病机为虚实夹杂,本虚属肝、脾、肾三脏脏腑功能失调,从而影响体内水湿代谢,而致水湿停聚,日久致瘀,导致痰、湿、瘀等病理产物形成,影响女性的肾-天癸-冲任-胞宫生殖轴的功能,是PCOS最为常见的病因病机。本文将以“肝脾肾三脏脏腑功能为本,痰湿、瘀血阻滞为标”为思路,在贺小林名老中医学术思想影响下,对PCOS的病因病机作一详细阐释,并将贺小林教授治疗本病的临床用药经验进行简要归纳分享。
Polycystic ovary syndrome(PCOS)is one of the most common endocrine and metabolic diseases in women of puberty and reproductive age.It has the characteristics of complicated etiology and pathogenesis and diverse clinical manifestations.It not only has obvious short-term clinical effects,but also is a high-risk cause of a variety of high-incidence diseases.TCM understanding of this disease can be analyzed from many angles,not only from the zang-fu meridian theory for syndrome differentiation and treatment,but also from phlegm,dampness,blood stasis and other pathological products for syndrome differentiation analysis,the author followed the outpatient department of He Shi,from the perspective of zang-fu syndrome differentiation,combined with phlegm,dampness,blood stasis and other pathological products for simple analysis of this disease.He thinks:of polycystic ovary syndrome according to TCM theory,combining with the history,symptoms,signs,such as concluded,phlegmy wet folder stasis syndrome is the most common,and more typical symptoms,pathogenesis factors,this virtual belongs to the liver,spleen,kidney three dirty viscera function disorder,which affects the metabolism of water wet,and the wet together,time will cause blood stasis,phlegm,dampness,blood stasis etc.The formation of pathological product,it affects the function of the reproductive axis of Kidney,Tiangui,Chongren and cell palace in women,and is the most common etiology and pathogenesis of PCOS.In this paper,the etiology and pathogenesis of PCOS will be explained in detail under the influence of He Xiaolin's academic thought of TCM,based on the function of liver,spleen and kidney,with phlegm-dampness and blood stasis as the standard,professor He Xiaolin's clinical experience in the treatment of this disease was briefly summarized and shared.
