摘要
目的探讨早期幽门后置管喂养联合支气管镜吸痰治疗卒中相关性肺炎(SAP)的临床疗效。方法选取2019年1月至2021年1月本院收治的98例SAP患者作为研究对象,按照随机化原则分为治疗组与对照组,每组49例。对照组给予常规抗感染对症治疗,治疗组给予幽门后置管喂食联合气管镜吸痰治疗,比较两组治疗前后美国国立卫生研究院卒中量表(NIHSS)、急性生理学和慢性健康状况评价Ⅱ(APACHEⅡ)、临床肺部感染评分(CPIS)评分、动脉血氧分压(PaO_(2))、炎症指标[降钙素原(PCT)、C反应蛋白(CRP)、白细胞(WBC)]及住院情况。结果治疗后,两组NIHSS、APACHEⅡ、CPIS评分均低于治疗前,且治疗组低于对照组,差异有统计学意义(P<0.05)。治疗后,两组PCT、CRP、WBC指标均低于治疗前,治疗组PaO_(2)指标高于治疗前,且治疗组PCT、CRP、WBC指标均低于对照组,PaO_(2)指标高于对照组,差异有统计学意义(P<0.05)。治疗组抗菌药物使用时间、住院时间均短于对照组,抗菌药物使用强度低于对照组,差异有统计学意义(P<0.05)。结论早期幽门后置管喂养联合支气管镜吸痰治疗卒中相关性肺炎疗效显著,可改善患者神经功能及健康状况,缩短抗菌药物使用时间、住院时间,值得临床推广应用。
Objective To investigate the clinical effect of early pyloric posterior tube feeding combined with bronchoscopic sputum aspiration in the treatment of stroke-associated pneumonia(SAP).Methods 98 patients with SAP admitted to our hospital from January 2019 to January 2021 were selected as research subjects,and they were randomly divided into treatment group and control group,with 49 cases in each group.The control group was given routine anti-infection symptomatic treatment,and the treatment group was given pyloric posterior tube feeding combined with tracheal sputum aspiration.National Institutes of Health stroke scale(NIHSS),acute physiology and chronic health status evaluationⅡ(APACHEⅡ),clinical pulmonary infection score(CPIS),PaO_(2),inflammation index Inflammatory markers(procalcitonin[PCT],C-reactive protein[CRP],white blood cell[WBC])before and after treatment and hospitalization were compared between the two groups.Results After treatment,NIHSS,APACHEⅡ,CPIS scores in the two groups were lower than before treatment,and the treatment group was lower than the control group,the difference was statistically significant(P<0.05).After treatment,PCT,CRP and WBC indexes of the two groups were lower than those before treatment,PaO_(2) index of the treatment group was higher than that before treatment,and PCT,CRP and WBC indexes of the treatment group were lower than those of the control group,PaO_(2) index was higher than that of the control group,the differences were statistically significant(P<0.05).The duration of antibiotic use and hospital stay in the treatment group were shorter than those in the control group,and the intensity of antibiotic use was lower than that in the control group,and the differences were statistically significant(P<0.05).Conclusion Early pyloric posterior tube feeding combined with bronchoscopy sputum aspiration has significant efficacy in the treatment of stroke-related pneumonia,which can improve the neurological function and health status of patients,shorten the use time of anti
