摘要
新型冠状病毒(Severe Acute Respiratory Syndrome Coronavirus 2,SARS-CoV-2)是属于冠状病毒科、β冠状病毒属、套式病毒目的线性单股正链核糖核酸病毒,血管紧张素转换酶(Angiotensin-converting Enzyme,ACE)2作为SARS-CoV-2感染机体的重要受体之一,直接或间接影响着新型冠状病毒感染(Coronavirus Disease 2019,COVID-19)的发生发展,调节ACE2已成为治疗COVID-19的切入点。因此,文章基于ACE2探讨应用中医药从肺肾防治COVID-19,以望得到防治该病的新启示。中医学认为,SARS-CoV-2属于“湿毒疫疠之邪”,通过分析现有研究发现,ACE2与COVID-19的发生发展息息相关,应用中医药从肺肾治疗COVID-19可调节机体ACE2的表达,进而起到改善细胞炎症反应、增强机体抵抗力、调节患者内环境稳态等正面调控作用。肺肾金水相生,与呼吸运动密切相关,大量研究表明,通过中医药的介入治疗能调节ACE2、改善肺肾之功能,起到固卫外之门户、补先天之正气等作用。多种中药成分可通过干预ACE2改善多脏器功能,黄芪、人参等均表现出了对ACE2的调控作用,加之ACE2作为SARS-CoV-2的重要靶点在肺肾高表达,因此,基于ACE2探讨从肺肾治疗COVID-19具有重大的临床价值。在COVID-19患者的治疗,以及未发病者及病后的日常调护方面,中医药皆具独特优势,结合中西医诊疗优势防治COVID-19,甚至研发治疗COVID-19的药物,已成为COVID-19防治的重要方向。
Novel coronavirus(SARS-CoV-2)is a linear single-stranded plus strand RNA virus belonging to the family coronavirus,the genus β coronavirus,and the order of the suite virus.Angiotensin-converting enzyme 2(ACE2),as one of the important receptors of novel coronavirus infection,directly or indirectly affects the occurrence and development of novel coronavirus pneumonia(COVID-19).Regulation of ACE2 has emerged as an entry point for COVID-19 treatment.Therefore,based on ACE2,the paper discusses the application of traditional Chinese medicine in the prevention and treatment of COVID-19 from lung and kidney,hoping to obtain new enlightenment on the prevention and treatment of the disease.According to traditional Chinese medicine,the novel coronavirus belongs to the evil of wet virus-pestilence.Through analysis of existing studies,it is found that ACE2 is closely related to the occurrence and development of COVID-19.The application of traditional Chinese medicine in the treatment of COVID-19 from lung and kidney can regulate the expression of ACE2 in the organism.Thus,it plays a positive regulatory role in improving cellular inflammatory response,enhancing body resistance,regulating patients’homeostasis and so on.The metal of lungs and the water of kidneys are mutually generated,which is closely related to respiratory movement.A large number of studies have shown that the interventional therapy of traditional Chinese medicine can regulate ACE2,improve the functions of the lungs and kidneys,and play a role in strengthening the external health and reinforcing the innate healthy Qi(气).A variety of TCM medicine ingredients can improve the function of multiple organs by interfering with ACE2.Huangqi(Astragalus),Renshen(Ginseng)and so on all show regulatory effects on ACE2,and ACE2 is highly expressed in lung and kidney as an important target of SARS-CoV-2.Therefore,exploring lung and kidney treatment of COVID-19 based on ACE2 is of great clinical value.Traditional Chinese medicine has its unique advantages in the treatmen
