摘要
目的探讨对妊娠期高血压患者采用肾脏超声与生化检查对评估肾脏功能受损的价值。方法选择高邮市中医医院2020年1月—2022年12月收治的妊娠期高血压患者150例为研究对象,根据是否发生急性肾损伤分为损伤组(n=76)与对照组(n=74),比较两组肾内动脉阻力指数(RI)、尿素氮(BUN)、肌酐(Cr)、尿酸(UA)水平,另根据能量多普勒超声(PDU)评分将急性期患者分为1分、2分、3分3个亚组,比较3组肾脏超声与生化检查结果。结果损伤组RI(0.73±0.06)、BUN(4.62±0.92)mmol/L、Cr(73.82±11.58)umol/L、UA(342.15±41.50)umol/L均高于对照组,差异有统计学意义(t=14.396、8.637、9.176、6.412,P<0.05)。1分组RI(0.78±0.06)、BUN(5.26±0.74)mmol/L、Cr(82.26±12.07)umol/L、UA(378.92±33.27)umol/L,高于2分组,且2分组高于3分组,差异有统计学意义(t=3.623、2.956、2.125、3.002、4.260、2.936、4.538、2.822,P<0.05)。结论妊娠期高血压肾功能损伤患者肾脏超声与生化检测指标存在明显异常,且损伤越严重该类指标变化越显著。
Objective To explore the value of renal ultrasonography and biochemical examination in evaluating renal function impairment in patients with hypertension during pregnancy.Methods 150 patients with gestational hypertension admitted to Gaoyou Traditional Chinese Medicine Hospital from January 2020 to December 2022 were selected as the research subjects.They were divided into an injury group(n=76)and a control group(n=74)based on whether acute kidney injury occurred.The renal artery resistance index(RI),urea nitrogen(BUN),creatinine(Cr),and uric acid(UA)levels of the two groups were compared.The acute phase patients were also divided into 1 and 2 points based on the power Doppler ultrasound(PDU)score Divide into three subgroups and compare the results of renal ultra-sound and biochemical examinations among the three groups.Results The RI(0.73±0.06),BUN(4.62±0.92)mmol/L,Cr(73.82±11.58)umol/L,and UA(342.15±41.50)umol/L in the injury group were all higher than those in the con-trol group,the difference was statistically significant(t=14.396,8.637,9.176,6.412,P<0.05).The RI(0.78±0.06),BUN(5.26±0.74)mmol/L,Cr(82.26±12.07)umol/L,and UA(378.92±33.27)umol/L in group 1 were higher than those in group 2,and group 2 was higher than group 3,the difference was statistically significant(t=3.623,2.956,2.125,3.002,4.260,2.936,4.538,2.822,P<0.05).Conclusion There are obvious abnormalities in renal ultrasound and biochemical detection indicators in patients with hypertensive renal injury during pregnancy,and the more serious the injury,the more significant the changes of these indicators.
