摘要
目的探究联合应用中医正骨与夹板外固定术治疗桡骨远端骨折的效果与预后恢复情况,旨在为其治疗提供更丰富的参考依据。方法选桡骨远端骨折患者110例(研究时间:2018年1月-2021年1月),按照计算机随机分配法分成参照组和研究组,各55例。参照组采用夹板外固定术治疗、研究组采用夹板外固定+中医正骨治疗。比对两组治疗效果、NRS评分、肿胀指数、腕关节消肿时间、疼痛缓解时间、骨折愈合时间及不良反应发生情况。结果(1)对照组显效29例、有效18例、无效8例,总有效率85.45%(47/55),观察组37例、16例、2例,总有效率为96.36%(53/55)。研究组治疗总有效率高于参照组,差异显著(χ^(2)=3.960,P=0.047)。(2)治疗前,组间对比,无差异(P>0.05);治疗后,NRS评分、肿胀指数降低,且研究组NRS评分、肿胀指数低于参照组,差异显著(P<0.05)。(3)研究组腕关节消肿时间、疼痛缓解时间、骨折愈合时间低于参照组,差异显著(P<0.05)。(4)研究组不良反应发生率低于参照组,差异显著(χ^(2)=4.274,P=0.039)。结论桡骨远端骨折患者行中医正骨联合夹板外固定治疗,疗效肯定,能提高腕关节恢复效果,缩短愈合时间,改善预后,值得优选。
Objective To explore effect and prognosis recovery of combined application of traditional Chinese medicine bone setting and splint external fixation in treatment of distal radius fractures,to provide richer reference basis for treatment.Methods The paper chose 110 patients with distal radius fractures(study period:from January 2018 to January 2021),and divided them into reference group and study group randomly,with 55 cases in each group.Reference group was treated with splint external fixation,while research group with splint external fixation and traditional Chinese medicine bone setting.Treatment effect,NRS scores,swelling index,wrist joint swelling reduction time,pain relief time,fracture healing time,and incidence of adverse reactions were compared between two groups.Results①Control group had 29 cases with significant effect,18 cases of being effective,and 8 cases with ineffective effect,with total effective rate of 85.45%(47/55).Observation group had 37 cases,16 cases,and 2 cases,with total effective rate of 96.36%(53/55).Total effective rate in study group was higher than reference group,with significant difference(χ^(2)=3.960,P=0.047).②Before treatment,there was no difference between groups(P>0.05).After treatment,NRS scores and swelling index decreased,NRS scores and swelling index in study group were significantly lower than reference group(P<0.05).③The study group had significantly shorter wrist joint swelling reduction time,pain relief time,and fracture healing time than control group(P<0.05).④Incidence of adverse reactions in study group was 5.45%(3/55),that was 18.18%(10/55)in reference group.Incidence of adverse reactions in study group was significantly lower than reference group(χ^(2)=4.274,P=0.039).Conclusion Traditional Chinese medicine bone setting combined with external fixation with splints can achieve definite curative effect for patients with distal radius fractures,can improve recovery effect of wrist joint,shorten healing time and improve prognosis,which is worthy of choice.
