摘要
目的:分析创伤性胫腓骨骨折外固定术后创口感染病原菌分布情况及耐药性.方法:选取我院住院部2021年1月—2023年1月收治创伤性胫腓骨骨折外固定术后创口感染患者60例,对患者创口感染病原菌分布情况以及耐药性进行分析.结果:60例患者共检出72株致病菌,革兰阳性菌共检出42株(58.33%),革兰阳性菌中金黄色葡萄球菌占比最高,为34.72%;革兰阴性菌共检出28株(38.89%),检出真菌2株(2.78%);革兰阴性菌中大肠埃希菌占比最高,为16.67%,铜绿假单胞菌为第二,占比为12.50%;25株金黄色葡萄球菌对青霉素、红霉素的耐药率最高,分别为92.00%、80.00%,对替考拉宁、万古霉素以及利奈唑胺的耐药率为0;主要革兰阴性菌中,铜绿假单胞菌对氨苄西林、磺胺甲唑/甲氧氧苄啶头孢以及庆大霉素的耐药率最高,为77.78%,大肠埃希菌对磺胺甲唑/甲氧苄啶头孢的耐药率最高,为100.00%,革兰阴性菌中最主要的病菌对美罗培南以及亚胺培南的耐药性均为0.结论:创伤性胫腓骨骨折外固定术后创口感染中以金黄色葡萄球菌、铜绿假单胞菌以及大肠埃希菌最为常见,临床应针对患者具体的病原菌分布情况,针对性用药.
Objective:To analyze the distribution and drug resistance of pathogenic bacteria in wound infection after external fixation of traumatic tibiofibular fracture.Methods:A total of 60 patients with wound infection after traumatic tibiofibula fracture were admitted to the inpatient department of our hospital from January 2021 to January 2023,and the pathogen distribution and drug resistance of patients were analyzed.Results:A total of 72 strains of pathogenic bacteria were detected in 60 patients,and 42 strains of Gram-positive bacteria were detected(58.33%).Staphylococcus aureus accounted for the highest proportion of Gram-positive bacteria,accounting for 34.72%.A total of 28 strains of Gram-negative bacteria(38.89%)and 2 strains of fungi(2.78%)were detected.Among Gram-negative bacteria,Escherichia coli accounted for the highest proportion(16.67%),followed by Pseudomonas aeruginosa(12.50%).The resistance rates of 25 strains of Staphylococcus aureus to penicillin and erythromycin were the highest,which were 92.00%and 80.00%,respectively,and the resistance rates to teicoplanin,vancomycin and linezolid were 0.Among the main Gram-negative bacteria,Pseudomonas aeruginosa had the highest resistance rate to ampicillin,sulfamethoxazole/trimethoprim cephalosporin and gentamicin,which was 77.78%.Escherichia coli had the highest resistance rate to sulfamethoxazole/trimethoprim cephalosporin,which was 100.00%.The resistance of the most important bacteria in Gram-negative bacteria to meropenem and imipenem was O.Conclusion:Staphylococcus aureus,Pseudomonas aeruginosa and Escherichia coli are the most common wound infections after external fixation of traumatic tibiofibular fractures.Clinically,targeted medication should be targeted at the specific pathogen distribution of patients.
