摘要
目的:观察即刻种植修复联合牙周基础治疗在慢性牙周炎患者中的应用效果。方法:选取2020年3月至2021年1月该院收治的80例慢性牙周炎患者进行前瞻性研究,按照随机数字表法将其分为对照组和观察组各40例。对照组予以牙周基础治疗,观察组在对照组基础上联合即刻种植修复治疗。比较两组治疗前和治疗后6个月牙周指标[探诊深度(PD)、菌斑指数(PLI)、出血指数(BI)、附着丧失(AL)]水平、龈沟液炎性因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)]水平和红色美学指数(PES)评分。结果:治疗后6个月,两组PLI、BI评分和PD、AL值均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);两组龈沟液TNF-α、IL-1β、IL-6水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);两组PES评分均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05)。结论:即刻种植修复联合牙周基础治疗应用于慢性牙周炎患者可提高PES评分,降低牙周指标和龈沟液炎性因子水平,效果优于单纯牙周基础治疗。
Objective:To observe application effects of immediate implant restoration combined with periodontal non-surgical treatment in patients with chronic periodontitis.Methods:A prospective study was conducted on 80 patients with chronic periodontitis admitted to this hospital from March 2020 to January 2021.They were divided into control group and observation group according to the random number table method,40 cases in each.The control group was treated with periodontal non-surgical treatment,while the observation group was treated with immediate implant restoration on the basis of that of the control group.The periodontal index levels[probing depth(PD),plaque index(PLI),bleeding index(BI),attachment loss(AL)],the inflammatory factor levels[tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),interleukin-6(IL-6)],and the pink aesthetic score(PES)were compared between the two groups before and 6 months after the treatment.Results:6 months after the treatment,the PLI and BI scores as well as PD and AL values of the two groups were lower than those before the treatment,those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The levels of TNF-α,IL-1βand IL-6 in the two groups were lower than those before the treatment,those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Further,the PES scores of the two groups were higher than those before the treatment,that in the observation group was higher than that in the control group,and the differences were statistically significant(P<0.05).Conclusions:Immediate implant restoration combined with periodontal non-surgical treatment for the patients with chronic periodontitis can improve the PES scores and reduce the periodontal index levels and the inflammatory factor levels.Moreover,it is superior to single periodontal non-surgical treatment.
