摘要
目的:分析温针灸联合经皮椎体成形术(PVP)在椎体压缩性骨折(VCFs)患者中的效果。方法:选取2019年4月─2022年4月在上杭县中医院诊治的204例VCFs患者为研究对象,按随机数表法将其分为两组,各102例。两组均行PVP治疗,对照组于PVP术后行抗感染、康复训练等对症处理,观察组于对照组基础上加以温针灸,连续治疗4周。比较两组骨折愈合情况、骨密度、疼痛程度、腰椎功能、日常生活能力、生活质量及并发症发生情况。结果:治疗4周后,两组腰椎L_(2~4)骨密度、椎体前缘高度、日常生活活动能力量表(ADL)评分较治疗前高,伤椎Cobb角、视觉模拟疼痛评估量表(VAS)评分、Oswestry功能障碍指数(ODI)评分较治疗前低,观察组腰椎L_(2~4)骨密度、椎体前缘高度、ADL评分较对照组高,伤椎Cobb角、VAS评分、ODI评分较对照组低,差异有统计学意义(P<0.05)。治疗4周后,两组生活质量综合评定问卷(GQOLI-74)各维度评分均高于治疗前,观察组GQOLI-74各维度评分均高于对照组,差异有统计学意义(P<0.05)。两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:温针灸联合PVP治疗VCFs患者效果显著,能够促进骨折愈合,提高骨密度,减轻疼痛,促进腰椎功能改善,提升日常生活能力,提高生活质量,且无严重并发症。
Objective:To analyze the effect of warm acupuncture combined with percutaneous vertebroplasty(PVP)in patients with vertebral compression fractures(VCFs).Method:A total of 204 VCFs patients diagnosed and treated in Shanghang County Hospital of Traditional Chinese Medicine from April 2019 to April 2022 were selected as the research objects,and they were divided into two groups according to random number table method,with 102 cases in each group.Both groups received PVP treatment,the control group received anti-infection,rehabilitation training and other symptomatic treatment after PVP,and the observation group received warm acupuncture on the basis of the control group,continuous treatment for 4 weeks.Fracture healing,bone density,pain,lumbar function,daily living ability,quality of life and complications were compared between the two groups.Result:After 4 weeks of treatment,the L_(2-4) bone mineral density,anterior vertebral height,and the score of the ability to perform activities of daily living scale(ADL)were higher in both groups than those before treatment,while the Cobb angle of the injured vertebra,visual analogue pain assessment scale(VAS)score and Oswestry disability index(ODI)score were lower than those before treatment,the L_(2-4) bone mineral density,anterior vertebral height and ADL score of the lumbar spine in the observation group were higher than those in the control group,while the Cobb angle of the injured vertebra,VAS score and ODI score were lower than those in the control group,the differences were statistically significant(P<0.05).After 4 weeks of treatment,the scores of generic quality of life inventory-74(GQOLI-74)in all dimensions of the two groups were higher than those before treatment,and the scores of GQOLI-74 in all dimensions of the observation group were higher than those of the control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion:Warm acupuncture combined
