摘要
目的X线与磁共振成像(magnetic resonance imaging,MRI)对脊柱压缩性骨折的诊断效能分析。方法收集江苏省启东市中医院2020年1月—2022年6月接诊的69例疑似脊柱压缩性骨折患者,均进行X线、MRI检查,以《骨质疏松性椎体压缩性骨折诊疗与管理专家共识》中脊柱压缩性骨折诊断标准为金标准,分析X线与MRI检查对该病的诊断价值,分析其影像学特征。结果金标准结果:69例疑似患者中阳性61例、阴性8例;X线检查结果:真阳51例、假阳2例、真阴6例、假阴10例;MRI诊断结果:真阳52例、假阳3例、真阴5例、假阴9例;联合检查:真阳60例、假阳0例、真阴8例、假阴1例。联合检查的准确性(98.55%)、敏感性(98.36%)高于X线检查,差异有统计学意义(P<0.05);两种检查方式诊断特异性比较,差异无统计学意义(P>0.05);联合检查的准确性(98.55%)、敏感性(98.36%)高于MRI检查,差异有统计学意义(P<0.05),联合诊断诊断特异性与MRI检查比较,差异无统计学意义(P>0.05)。结论X线检查是诊断外伤性脊柱压缩性骨折的基础检查方法,联合X线与MRI检查对椎体骨折诊断的敏感度和准确性更高。
Objective To analyze the diagnostic efficacy of X-ray and magnetic resonance imaging(MRI)on spinal compression fractures.Methods Sixty-nine patients with suspected spinal compression fractures who were treated at Qidong Hospital of Traditional Chinese Medicine from January 2020 to June 2022 were collected and all underwent X-ray and MRI examinations.The diagnostic value of X-ray and MRI examinations for this disease was analyzed using the spinal compression fracture diagnostic criteria in the Expert Consensus on the Treatment and Management of Osteoporotic Vertebral Compression Fractures as the gold standard,and the imaging features were analyzed.Results Gold standard results:Among 69 suspected patients,61 were positive and 8 were negative;X-ray examination results:51 cases were true positive,2 cases were false positive,6 cases were true negative,and 10 cases were false negative;MRI diagnosis results:52 cases of true positive,3 cases of false positive,5 cases of true negative,and 9 cases of false negative;Joint examination:60 cases of true positive,0 cases of false positive,8 cases of true negative,and 1 case of false negative.The accuracy(98.55%)and sensitivity(98.36%)of joint examination were higher than those of X-ray examination,and the difference was statistically significant(P<0.05).There was no statistically significant difference in diagnostic specificity between the two examination methods(P>0.05).The accuracy(98.55%)and sensitivity(98.36%)of joint examination were higher than those of MRI examination,and the difference was statistically significant(P<0.05).The specificity of joint diagnosis was not significantly different from that of MRI examination(P>0.05).Conclusion X-ray examination is the basic examination method for the diagnosis of traumatic spinal compression fractures,and the sensitivity and accuracy of combined X-ray and MRI examination for the diagnosis of vertebral fractures are higher.
