摘要
目的对比研究不同血管吻合方法血管内径狭窄的程度。方法选取新鲜三黄鸡鸡翅72个,根据不同血管吻合法分为三组,每组各24个鸡翅,显露鸡翅翅中动脉并剪断,血管断端分别采用单纯缝合法、水平褥式缝合法及四定点水平褥式缝合法吻合,并分别使用孔径规测量血管断端吻合前后血管内径。结果单纯缝合组吻合前血管内径为(0.92±0.15)mm,吻合后测量值为(0.95±0.14)mm,P<0.05,差异有统计学意义,血管吻合后断端扩张约3.52%±2.74%;水平褥式缝合组血管吻合前内径测量值为(0.89±0.11)mm,吻合后测量值为(0.61±0.07)mm,P<0.05,差异有统计学意义,血管吻合后断端狭窄约31.56%±2.88%;四定点水平褥式缝合组血管内径吻合前测量值为(0.96±0.12)mm,吻合后测量值为(0.75±0.09)mm,P<0.05,差异有统计学意义,血管吻合后断端狭窄21.78%±2.13%。三组缝合方法狭窄率两两对比P<0.05,差异有统计学意义。结论血管单纯缝合法吻合后,吻合口轻度扩张,四定点水平褥式缝合法及全水平褥式缝合法均可导致血管狭窄,可根据个人习惯选择合适可靠的血管吻合方法用于显微外科手术。
Objective To compare the degree of stenosis of vascular diameter by dfferent vascular anastomosis.Methods Seventy-two fresh Sanhuang chicken wings were selected and divided into simple suture group,level mattress suture group,and four fixed-point level mattress suture group.The chicken wing middle wing artery was exposed and cut off,the diameter of the vessel before and after the stapling was measured using a diameter gauge.Results In the suture group,the diameter of the vessel before anastomosis was(0.92±0.15)mm,and the measured value after anastomosis was(0.95±0.14)mm,P<0.05,the difference was statistically significant.The diameter of the vessel after anastomosis was 3.52%±2.74%,and the diameter of the vessel after anastomosis was(0.89±0.11)mm in the level mattress suture group,P<0.05,the difference was statistically significant.In the four-point level mattress suture group,the measured value of vessel diameter before anastomosis was(0.96±0.12)mm and after anastomosis was(0.75±0.09)mm,P<0.05 with statistically significant difference.The stenosis of the broken end after anastomosis was about 21.78%±2.13%;stenosis rate of the three groups was compared with P<0.05,and the difference was statistically significant.Conclusion The anastomosis is slightly dilated after the simple suture of blood vessels.Both the four-point level mattress suture method and the whole level mattress suture method can lead to vascular stenosis.Appropriate and reliable vascular anastomosis method can be selected for microsurgery according to personal habits.
