摘要
目的分析掌侧动脉静脉化血管吻合在末节断指再植中的应用效果。方法选取2017年2月至2020年2月郑州市骨科医院收治的66例(74指)拟行末节断指再植的手指末节离断患者作为研究对象,按照不同治疗方法将其分为试验组38例(42指)和对照组28例(32指),试验组患者于断指再植术中行掌侧动脉静脉化血管吻合,对照组患者于断指再植术中行背侧静脉血管吻合,对比观察两组患者断指成活率、患指功能恢复情况以及并发症发生情况。结果试验组患者断指成活率为90.48%,与对照组患者的断指成活率78.13%无明显差异(χ^(2)=2.189,P=0.139);术后12个月,试验组患者患指功能恢复优良率为85.71%,明显高于对照组患者的患指功能恢复优良率65.63%(χ^(2)=4.142,P=0.042);试验组患者术后并发症发生率为16.67%,与对照组患者的术后并发症发生率34.38%无明显差异(χ^(2)=3.090,P=0.079)。结论与末节断指再植术中行背侧静脉血管吻合相比,采用掌侧动脉静脉化血管吻合的患者术后患指功能恢复效果更好。
Objective To analyze the application effect of palmar arterio⁃venolization anastomosis in the replanta⁃tion of severed distal fingers.Methods 66 patients with severed distal fingers(74 fingers involved)admitted into Zhengzhou Orthopedic Hospital between February 2017 and February 2020 for replantation of severed distal fingers were enrolled as research subjects,and then divided into study group(n=38,involving 42 fingers)and control group(n=28,involving 32 fingers)based on different treatments they received.Patients in the study group underwent palmar arterio⁃venolization anastomosis during the replantation of severed finger,while patients underwent dorsal venous anastomosis during the replantation of severed fingers in the control group.The survival rate of severed fingers,functional recovery of the affected finger and occurrence of complications were compared between the two groups.Results The survival rate of severed fingers was 90.48%in the study group,which showed no significant difference compared with the corresponding 78.13%in the control group(χ^(2)=2.189,P=0.139).12 months after surgery,the excellent and good rate of functional recovery of the affected fingers was 85.71%in the study group,being obviously higher than the corresponding 65.63%in the control group(χ^(2)=4.142,P=0.042).The incidence of postoperative complications was 16.67%in the study group,which showed no significant difference compared with the corresponding 34.38%in the control group(χ^(2)=3.090,P=0.079).Conclusion Compared with dorsal venous anastomosis during the implantation of severed distal fingers,the palmar arterio⁃venolization anastomosis can realize better functional recovery of the affected fingers.
