摘要
目的探讨T2DM合并不典型铁沉积症患者肝病理与各指标的相关性。方法选取2020年10月至2021年10月于湖北医药学院附属国药东风总医院内分泌代谢科住院收治的T2DM合并铁蛋白(SF)升高行肝活检患者20例,按照肝病理结果分为G1-2、S0-2的轻度组(n=10),G3、S1-3的中度组(n=7),G4、S2-4的重度组(n=3)。收集患者一般资料,读取肝病理报告,Pearson相关分析肝病理GS分级与其他指标的相关性。结果16例患者肝实质细胞不同程度含铁血黄素染色物质沉积。与轻度组比较,中度组年龄、FPG、谷草转氨酶(AST)及SF升高(P<0.05),重度组年龄、AST、血清铁及SF升高(P<0.05)。与中度组比较,重度组AST、SF升高(P<0.05)。Pearson相关分析显示,肝病理GS分级与AST、SF呈正相关(P<0.05)。结论SF升高的T2DM患者肝活检多数可见肝实质细胞内含铁血黄素沉积阳性,血清SF越高,肝纤维化程度越重。
Objective To investigate the relationship between liver pathology and various indexes in type 2 diabetes mellitus(T2DM)patients with atypical iron deposition.Methods A total of 20 patients diagnosed with T2DM combined with elevated serum ferritin(SF)who underwent live biopsy in the Department of Secretion and Metabolism of our hospital were enrolled in this study from October 2020 to October 2021.All the patients were divided into three groups according to the results of liver disease:mild:G1-2,S0-2(mild group,n=10),moderate:G3,S1-3(moderate group,n=7),and severe:G4,S2-4(severe group,n=3).General data of patients and liver Pathology reports were analyzed.Pearson correlation analysis was conducted to evaluate the correlation between GS grades and other indicators of liver disease.Results There were hemosiderin stain deposition with different degrees in liver parenchyma cells of 16 patients.Compared with mild group,age,FPG,AST and SF were increased in moderate group(P<O.05),and age,AST,SI and SF were increased in severe group(P<0.05).Compared with moderate group,AST and SF were increased in severe group(P<0.05).Pearson correlation analysis showed that GS grading of liver disease was positively correlated with AST and SF(P<0.05).Conclusion In the liver biopsy of T2DM patients with elevated SF,hemosiderin stain deposition could be seen in most of the liver parenchymal cells.The higher the serum SF level,the more severe the degree of liver fibrosis.
