肠道菌群-胆汁酸途径对2型糖尿病的影响

摘要 目的综述肠道菌群-胆汁酸通路对2型糖尿病(T2DM)发生发展及防治的研究进展,为基于该通路的糖尿病防治提供文献参考,为开发治疗糖尿病的创新治疗方法提供思路。方法通过查阅国内外相关文献,我们综述了肠道菌群与T2DM的相关性、胆汁酸和T2DM相关性、肠道菌群与胆汁酸代谢的相关性,并重点论述了肠道菌群-胆汁酸通路在T2DM发生发展及防治中的研究进展,强调该路径对血糖调节方面的局限性和挑战,并探讨如何基于该路径开发防治T2DM的创新方法。结果与结论肠道菌群-胆汁酸通路可介导T2DM的发生发展。肠道菌群可通过一系列酶促反应,影响初级胆汁酸转变为次级胆汁酸,随后通过法尼醇X受体(FXR)、G蛋白偶联胆汁酸受体5(TGR5)调节信号传导,影响宿主的代谢途径。FXR和TGR5受体已成为糖尿病等代谢疾病研究的新靶点,开发肠道特异性FXR拮抗剂和TGR5激动剂可能是未来治疗T2DM的一个很有前途的方法,然而其长期效果值得进一步评估。 Objective This paper reviewed the research progress of intestinal microbiota-bile acid pathway on the occurrence,development,prevention and treatment of type 2 diabetes mellitus(T2DM)to provide references for the prevention and treatment of diabetes mellitus based on this pathway and provide ideas for the development of innovative treatment for diabetes mellitus.Method By reviewing relevant literatures at home and abroad,we reviewed the correlation between intestinal flora and T2DM,as well as the correlation between intestinal flora and bile acid metabolism,focusing on the research progress of intestinal flora-bile acid pathway in the occurrence,development and prevention of T2DM and emphasized the limitations and challenges of this pathway in blood glucose regulation to explore how to develop an innovative approach for prevention and treatment of T2DM based on this pathway.Result and Conclusion The intestinal microbiota-bile acid pathway can mediate the occurrence and development of T2DM.Intestinal flora affects the transformation of primary bile acids into secondary bile acids through a series of enzymatic reactions,subsequently,fannitol X receptor(FXR)and G protein-coupled bile acid receptor 5(TGR5)modulate signal transduction and affect the metabolic pathway of the host.FXR and TGR5 receptors had become new targets for the study of metabolic diseases such as T2DM.Developing intestinal-specific FXR antagonists and TGR5 agonists may constitute a promising future approach for hyperglycemia treatment.However,the long-term effects of blocking intestinal FXR and activation of TGR5 merit future evaluation.
