摘要
目的探讨耳穴埋豆联合自拟中药方口服在中风后抑郁中的疗效及对心理状态、神经功能、血清指标、生活质量的影响,以期为临床治疗提供有效参考。方法选取张家口市中医院2019年2月—2021年6月期间收治的112例中风后抑郁症患者,根据采用随机数字表法将其分为对照组和治疗组,每组56例,两组均给予西医常规治疗,治疗组在以上基础给予耳穴埋豆联合自拟中药方口服治疗,两组治疗周期均为3个月。观察两组心理状态、神经功能、血清因子[脑源性神经营养因子(BDNF)、超敏C反应蛋白(hs-CRP)、皮质醇(Cro)]及生活质量的改善情况,并对其疗效及安全性进行客观评价。结果治疗后两组卒中后抑郁评估量表(PSDS)评分、汉密尔顿抑郁量表(HDRS)评分、美国国立卫生研究院量表(NIHSS)评分、BDNF、hs-CRP、Cro及日常生活能力量表(ADL)评分均得到显著改善(P<0.05),且治疗组改善效果优于对照组,差异有统计学意义(P<0.05);治疗组总有效率显著高于对照组(P<0.05),且不良反应发生率低于对照组(P<0.05)。结论耳穴埋豆联合自拟中药方用于中风后抑郁症患者中,其疗效显著,不仅能改善患者焦虑抑郁状态,提高神经功能,还能调节其血清因子水平,促进生活质量的提升,安全可靠。
Objective To explore the curative effect of auricular pressure combined with self-made TCM prescription on post-stroke depression and its influences on psychological state,nerve function,serum indexes and quality of life,in order to provide effective reference for clinical treatment.Methods A total of 112 patients with post-stroke depression admitted to Zhangjiakou Hospital of Traditional Chinese Medicine were enrolled between February 2019 and June 2021.According to the random number table method,they were divided into control group and treatment group,56 cases in each group.Both groups were treated with routine western medicine.On this basis,the treatment group was given auricular pressure combined with self-made TCM prescription.All were treated for 3 months.The improvement of psychological state,nerve function,serum factors[brain-derived neurotrophic factor(BDNF),high-sensitivity C-reactive protein(hs-CRP),cortisol(Cro)]and the quality of life in both groups were observed.The curative effect and safety were evaluated objectively.Results After treatment,the scores of Post-stroke Depression Rating Scale(PSDS),Hamilton Depression Rating Scale(HDRS)and National Institutes of Health Stroke Scale(NIHSS),BDNF,hs-CRP,Cro and scores of activity of daily living scale(ADL)in both groups were significantly improved(P<0.05),and the improvement effect was better in treatment group than that in the control group(P<0.05).The total response rate of the treatment group was significantly higher than that of the control group(P<0.05),and the incidence of adverse reactions was lower than that of the control group(P<0.05).Conclusion The treatment of auricular pressure combined with self-made TCM prescription has significant effect on patients with post-stroke depression.It can not only alleviate anxiety and depression,improve the nerve function,but also adjust the levels of serum factors and improve the quality of life,with high safety and reliability.
