摘要
目的:分析程序性死亡受体配体1(PD-L1)在结直肠癌组织中的表达及其意义。方法:选取2019年2月—2022年9月东莞市人民医院收治的50例结直肠癌患者作为研究对象,均进行PD-L1检测,分析PD-L1在结直肠癌组织以及癌间质的阳性表达情况及患者随访情况。结果:50例结直肠癌患者PD-L1阳性35例,阳性率为70.0%。PD-L1在中高分化、低分化、肿瘤直径≥5 cm、肿瘤直径<5 cm、溃疡型、存在淋巴转移、不存在淋巴转移、术前CEA升高结肠癌组织中的阳性表达率均高于癌间质,差异有统计学意义(P<0.05)。50例患者平均生存时间为(31.94±10.33)个月。死亡5例,均为中高分化患者,同时存在淋巴转移,其肿瘤直径>5 cm。结论:PD-L1在T细胞的介导细胞免疫方面仍存在研究空间,也可以作为单独的受体发挥作用,故而,在临床诊断过程中,检测PD-L1的情况,可以了解患者的具体情况,并为临床靶向药物的研究提供方案。
Objective:To analyze the expression and significance of programmed death receptor ligand 1(PD-L1)in colorectal cancer tissue.Methods:From February 2019 to September 2022,a total of 50 patients with colorectal cancer admitted to Dongguan People's Hospital were selected as the study subjects.All the patients were tested for PD-L1.The positive expression of PD-L1 in colorectal cancer tissues and cancer stroma and patient’s follow-up were analyzed.Results:Among the 50 colorectal cancer patients,35 cases were positive for PD-L1,and the positive rate was 70.0%.The positive expression rate of PD-L1 in colon cancer tissues with moderate and high differentiation,low differentiation,tumor diameter≥5 cm,tumor diameter<5 cm,ulcerative type,presence of lymphatic metastasis,absence of lymphatic metastasis,and increased preoperative CEA were higher than that in cancer stroma,and the difference was statistically significant(P<0.05).The average survival time of 50 patients was(31.94±10.33)months.Five cases died and they were all moderately and highly differentiated patients,with lymphatic metastases and tumor diameter>5 cm.Conclusion:There is still room for investigation regarding PD-L1 in the T cell-mediated cellular immunity,and it can also function as an individual receptor,so in the process of clinical diagnosis,the detection of PD-L1 can understand the specific situation of patients and provide a scheme for the research of clinical targeted drugs.
