摘要
目的探讨沙美特罗替卡松吸入粉雾剂联合高流量氧疗对慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)并发呼吸衰竭治疗效果。方法便利选取2020年2月—2022年2月兴化市第三人民医院接诊的72例COPD患者,根据入院先后顺序编号(1~72号),其中1~36号采用沙美特罗替卡松吸入粉雾剂吸入治疗,纳入对照组;37~72号采用沙美特罗替卡松吸入粉雾剂联合高流量氧疗,纳入观察组。治疗两周后比较两组疗效、肺功能指标及动脉血气指标。结果临床总有效率比较,观察组比对照组高(97.22%vs77.78%),差异有统计学意义(χ^(2)=4.571,P=0.033);治疗两周后,观察组FVC、FEV_(1)、PEFR水平均高于对照组,PaO_(2)、SaO_(2)值均比对照组高,PaCO_(2)值较对照组低,差异有统计学意义(P<0.05)。结论沙美特罗替卡松吸入粉雾剂联合高流量氧疗治疗COPD,可促进肺功能及血气指标改善,从而提高临床疗效。
Objective To investigate the therapeutic effect of salmeterol fluticasone inhalation powder nebulizer combined with high-flow oxygen therapy on the treatment of respiratory failure complicated by chronic obstructive pulmonary disease(COPD).Methods 72 COPD patients admitted to Xinghua Third People's Hospital from February 2020 to February 2022 were conveniently selected and numbered according to the order of admission(No.1-72).Among them,numbers 1-36 were treated with salmeterol fluticasone inhalation powder nebulizer and included in the control group,while numbers 37~72 were treated with salmeterol fluticasone inhalation powder nebulizer combined with highflow oxygen therapy and included in the observation group.After two weeks of treatment,the therapeutic effect,lung function index and arterial blood gas index were compared between the two groups.Results The total clinical efficiency was higher in the observation group than in the control group(97.22%vs 77.78%),the difference was statistically significant(χ^(2)=4.571,P=0.033).Two weeks after treatment,the levels of FVC,FEV_(1) and PEFR in the observation group were higher than those in the control group,PaO_(2) and SaO_(2) values were higher than those in the control group,and PaCO_(2) values were lower than those in the control group,the difference was statistically significant(P<0.05).Conclusion Salmeterol fluticasone inhalation powder nebulizer combined with high-flow oxygen therapy for COPD can promote the improvement of lung function and blood gas indexes,thus improving the clinical efficacy.
