慢性阻塞性肺疾病并发呼吸衰竭相关风险因素分析 被引量:4

Comparison of Adverse Reactions of Pegaspargase and L-asparaginase for Acute Lymphoblastic Leukemia in Children

摘要 目的探讨慢性阻塞性肺疾病(COPD)并发呼吸衰竭的相关风险因素。方法选择珠海市人民医院2014年5月至2015年10月收治的COPD患者84例,收集其临床资料,包括性别、年龄、吸烟支数、病史、1年内发作次数及实验室检查等指标,并按血气分析指标将所有患者分成并发呼吸衰竭组和非呼吸衰竭组。采用单因素和多因素分析比较2组患者的临床资料。结果 84例患者中,呼吸衰竭组28例(33.3%),非呼吸衰竭组56例(66.7%)。单因素分析显示,COPD并发呼吸衰竭与白细胞计数、C反应蛋白(C-reactive protein,CRP)水平、1年内发作次数及白蛋白水平有关。多因素分析显示,白细胞计数≥10×109,白蛋白<30g·L-1及CRP≥12mg·L-1为COPD患者并发呼吸衰竭的独立风险因素。结论感染及低蛋白血症为COPD患者并发呼吸衰竭的风险因素,对于COPD急性发作患者,应注意观察白细胞计数、CRP及血浆白蛋白等指标,采取必要措施避免呼吸衰竭的发生。 Objective T o compare th e ad v erse re a c tio n s of p eg a sp a rg a se and L -a sp a rag in a se in the treatment of acute lymphoblastic leukemia in children,and to provide a basis for clinical medi-cation. Methods Atotal of 118 child ren were t re a te d with L-a sp a rag in a se (2 8 2 case t ime s ) or pegaspargase(228 case times) according to the CCLG-ALL 2008 protocol,including remission induc-tion and delayed reinforcement programs. Blood routine, liver function, blood glucose, plasma pro-tein, coagulation function, serum amylase and urine amylase examinations were performed and changes in clinical manifestations and indicators were recorded in both groups. Adverse reactions were classified according to Common Terminology Criteria for Adverse Events version 3. 0. Re-sults The incidence of al lergic react ion in pegaspargase group was lower than that in L-asparagi- nase group(P = 0. Oil). No significant differences in the incidence of myelosuppression,hypergly-cemia, coagulation disorder, liver dysfunction and acute pancreatitis were found between the two groups(P〈0. 05). Conclusion Compared with L -a sparag inase , treatment with p eg a spargase can reduce the frequency of injection and the incidence of allergic reaction in children with acutelym phoblastic leukemia.
