摘要
【目的】探讨肋骨骨折合并其他部位骨折患者深静脉血栓(DVT)形成的危险因素。【方法】回顾性分析283例肋骨骨折合并其他部位骨折患者临床资料,根据患者术后是否发生DVT分为发生DVT组和未发生DVT组。比较两组患者临床资料、手术指标以及实验室指标,采用多因素Logistic回归分析患者术后发生DVT的独立危险因素。【结果】283例患者中105例(37.10%)患者在术后发生DVT,其中1例患者合并急性肺栓塞,无死亡病例。两组患者性别、胸部合并症、合并内科疾病、合并其他部位损伤等比较,差异无统计学意义(P>0.05);与未发生DVT组患者相比,发生DVT组患者年龄较大、肋骨骨折根数较多、双侧肋骨骨折以及合并多部位骨折(P<0.05)。两组患者术中失血量、是否术中输血比较,差异无统计学意义(P>0.05),但两组手术时间比较,差异有统计学意义(P<0.05)。与未发生DVT组患者相比,发生DVT组患者血红蛋白含量、凝血酶原时间减少,D-二聚体含量增加(P<0.05)。Logistic多因素回归分析显示:肋骨骨折根数、合并多部位骨折、凝血酶原时间及D-二聚体含量是肋骨骨折合并其他部位骨折患者发生DVT的独立危险因素。【结论】肋骨骨折合并其他部位骨折患者术后发生DVT的高危因素包括肋骨骨折根数、合并多部位骨折、凝血酶原时间及D-二聚体含量。
【Objective】To explore the risk factors of deep vein thrombosis(DVT)in patients with rib fracture and other fractures.【Methods】A retrospective analysis was conducted on the clinical data of 283 patients with rib fractures combined with fractures in other parts.The patients were divided into two groups based on whether DVT occurred after surgery:the group with DVT and the group without DVT.We compared clinical data,surgical indicators,and laboratory indicators between two groups of patients,and used multivariate logistic regression to analyze the independent risk factors for postoperative DVT in patients.【Results】Out of 283 patients,105(37.10%)developed DVT after surgery,with 1 patient complicated with acute pulmonary embolism and no deaths.There was no statistically significant difference between the two groups in terms of gender,chest comorbidities,comorbidities with internal medicine,and comorbidities with other injuries(P>0.05);Compared with the group without DVT,the group with DVT had older age,more rib fractures,bilateral rib fractures,and multiple site fractures(P<0.05).There was no statistically significant difference in intraoperative blood loss and whether intraoperative blood transfusion was performed between the two groups of patients(P>0.05),but the difference in surgical time between the two groups was statistically significant(P<0.05).Compared with the group without DVT,the group with DVT showed a decrease in hemoglobin content and prothrombin time,while the D-dimer content increased(P<0.05).Logistic multivariate regression analysis showed that the number of rib fractures,combined multiple site fractures,prothrombin time,and D-dimer content were independent risk factors for DVT in patients with rib fractures combined with other site fractures.【Conclusion】The high-risk factors for postoperative DVT in patients with rib fractures combined with other site fractures include the number of rib fractures,multiple site fractures,prothrombin time,and D-dimer content.
