摘要
目的探讨不同分期矽肺患者气道细菌微生物构成的差异。方法采用立意抽样方法,选择29例职业性矽肺病患者,13例健康受试者为研究对象,其中矽肺壹期患者6例(矽肺壹期组),矽肺贰期患者15例(矽肺贰期组),矽肺叁期患者8例(矽肺叁期组)。采集42例受试者痰液样本,采用16S rDNA测序技术检测痰液样品中细菌微生物,分析4组受试者细菌微生物的相对丰度差异,对健康受试者与矽肺患者组间存在差异的细菌微生物进行物种多样性指数(alpha多样性指数、beta多样性指数)差异分析;绘制受试者工作特征曲线(ROC)评估细菌微生物作为生物标记物的可行性。结果健康受试者与矽肺患者痰液样品中奈瑟菌属(Neisseria)、莫拉氏菌属(Moraxella)、葡萄球菌属(Staphylococcus)、棒状杆菌属(Corynebacterium)、狡诈球菌属(Dolosigranulum)、密螺旋体菌属(Treponema)相对丰度分别比较,差异均有统计学意义(P<0.05)。LEfSe分析结果显示,矽肺壹期组增加的物种包括Neisseria、奈瑟氏菌科(Neisseriaceae)、假单胞菌目(Pseudomonadales)和莫拉菌科(Moraxellaceae);矽肺贰期组增加的物种有芽孢杆菌科(Bacillacea)、芽孢杆菌属(Bacillus)、Staphylococcus、葡萄球菌科(Staphyloccaceae)和金黄色葡萄球菌(Staphylococcus_aureus)。ROC曲线分析结果显示,采用Neisseria相对丰度对矽肺壹期与矽肺贰期患者进行分期诊断的ROC曲线下面积为0.87(95%置信区间0.71~0.99),灵敏度100.0%,特异度73.3%,约登指数0.73,P=0.01。结论Neisseria可能与矽肺患者的疾病进程有关,并可能具备对矽肺分期进行区别的生物标记物的潜力。
Objective To investigate the composition in bacterial microbial composition in silicosis pa⁃tients with different stages.Methods 13 healthy subjects,29 patients with occupational silicosis,including 6 pa⁃tients with silicosis I,15 patients with silicosisⅡand 8 patients with silicosisⅢ.The sputum of four groups of subjects was collected,16S rDNA sequencing technology was used to detect bacterial microorganisms in the spu⁃tum samples,and the relative abundance of bacterial microorganisms and the difference of microbial diversity in⁃dex(alpha diversity index,beta diversity index)were analyzed;the subject working characteristic curve(ROC)was drawn to assess whether bacterial microorganisms have the potential of biomarkers.Results The relative abun⁃dance of Neisseria,Moraxella,Staphylococcus,Corynebacterium,Dolosigranulum and Treponema in sputum sam⁃ples of subjects(P values<0.05).The results of the LEfSe analysis showed that,the increased species in the sili⁃cosis I group include Pseudomonadales,Neisseriaceae,Moraxellaceae,and Neisseria.The additional species in⁃cluded in the silicosisⅡgroup were Staphyloccaceae Bacillaceae,Bacillus,Staphylococcus,and Staphylococ⁃cus_aureus.The ROC analysis showed that the area using Neisseria relative abundance of silicosisⅠand silicosisⅡwas 0.87(95%confidence 0.71~0.99),sensitivity=100.0%,specificity=73.3%,Youden index=0.73,P=0.01.Conclusion Neisseria may be related to the disease process in patients with silicosisis and have the po⁃tential as a biomarker for distinguishing the silicosis stage.
