摘要
目的:探讨平均红细胞体积(MCV)及血红蛋白(Hb)电泳在筛查中孕期孕妇地中海贫血中的临床应用价值。方法:回顾性分析2019年5月-2021年6月于宁德师范学院附属宁德市医院进行孕期检查的320例中孕期孕妇的临床资料,根据基因检测结果将124例地中海贫血患者划分为地贫组,将196例非地中海贫血患者划分为非地贫组,两组孕妇均接受MCV及Hb电泳检查,对比两组孕妇的检查结果。结果:地贫组的MCV阳性率及Hb电泳阳性率均高于非地贫组,差异均有统计学意义(P<0.05)。MCV及Hb电泳对地中海贫血诊断的敏感度分别为87.10%、83.06%,特异度分别为70.92%、72.45%,准确度分别为77.19%、76.56%,两者联合检测的敏感度、特异度及准确度分别为97.58%、94.39%、95.63%。联合检测的敏感度及特异度均高于单项检测,差异均有统计学意义(P<0.05)。结论:MCV及Hb电泳在中孕期孕妇地中海贫血筛查诊断中具有较高的敏感度及特异度,两者联合检测可提高诊断率。
Objective:To investigate the clinical application value of mean erythrocyte volume(MCV)and hemoglobin(Hb)electrophoresis in screening for thalassemia in pregnant women during second trimester.Method:The clinical data of 320 second trimester pregnant women who underwent pregnancy screening in Ningde Municipal Hospital of Ningde Normal University from May 2019 and June 2021 were retrospectively analyzed,and 124 thalassemia patients were classified as the thalassemia group and 196 non-thalassemia patients were classified as the non-thalassemia group according to the genetic test results,and both groups received MCV and Hb electrophoresis,and the test results of the two groups were compared.Result:The positive rates of MCV and Hb electrophoresis in the thalassemia group were higher than those in the non-thalassemia group,and the differences were statistically significant(P<0.05).The sensitivity of MCV and Hb electrophoresis for the diagnosis of thalassemia were 87.10%and 83.06%,the specificity was 70.92%and 72.45%,and the accuracy was 77.19%and 76.56%,respectively.The sensitivity,specificity and accuracy of the combined assay were 97.58%,94.39%and 95.63%,respectively.The sensitivity and specificity of the combined test were higher than those of the single test,and the differences were statistically significant(P<0.05).Conclusion:MCV and Hb electrophoresis have high sensitivity and specificity in screening and diagnosis of thalassemia in pregnant women in the second trimester,and the combined test can improve the diagnosis rate.
