屈曲体位不同维持时间对全膝关节置换术后早期膝关节恢复情况的影响

Influence of Different Maintenance Time of Flexion Position on Early Knee Joint Recovery after Total Knee Arthroplasty

摘要 目的:探讨屈曲体位不同维持时间对全膝关节置换术后早期膝关节恢复情况的影响。方法:选取2019年8月-2021年2月中国人民解放军联勤保障部队第九一〇医院收治的92例全膝关节置换术后患者,按照随机数字表法将其分为观察组和对照组。两组术后送返病房后均采取屈髋、屈膝30°体位,对照组46例维持24 h,观察组46例维持48 h。比较两组失血情况、膝关节恢复情况、肢体水肿程度及并发症发生率。结果:术后3 d观察组的血红蛋白(Hb)、红细胞压积(HCT)、红细胞计数(RBC)水平均高于对照组(P<0.05);术后3 d观察组的膝关节活动度高于对照组,视觉模拟评分法(VAS)评分低于对照组(P<0.05)。术后3 d观察组的大腿围、小腿围均低于对照组(P<0.05)。两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:膝关节置换患者术后维持屈曲体位48 h有助于减少患者失血,减轻患肢肿胀程度,改善膝关节活动度,缓解疼痛,且不增加术后并发症发生率。 Objective:To investigate the effect of different maintenance time of flexion position on the early knee joint recovery after total knee arthroplasty.Method:A total of 92 patients with total knee arthroplasty admitted to the 910th Hospital of Joint Logistics Support Force of Chinese People's Liberation Army from August 2019 to February 2021 were selected,they were divided into observation group and control group according to the random number table method.After returned to the ward after surgery,both groups took the position of hip flexion and knee bending at 30°,46 cases in the control group were maintained for 24 h,and 46 cases in the observation group were maintained for 48 h.The blood loss,knee joint recovery,degree of limb edema and the incidence of complications were compared between the two groups.Result:The levels of hemoglobin(Hb),hematocrit(HCT),red blood cell count(RBC)in observation group were higher than those in control group at 3 d after operation(P<0.05).At 3 d after operation,the range of motion of knee joint in observation group was higher than that in control group,and the visual analog scale(VAS)score was lower than that in control group(P<0.05).The thigh circumference and calf circumference in the observation group were lower than those in the control group at 3 d after operation(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion:Maintaining the flexion position for 48 h after knee arthroplasty can help reduce blood loss,reduce swelling of the affected limb,improve the range of motion of the knee joint,relieve pain,and does not increase the incidence of postoperative complications.
